For Health Care Professionals The most recent biologic product approved by the FDA for the treatment of psoriasis is Stelara (ustekinumab). Ustekinumab contains an antibody that’s produced in a laboratory and designed to bind to a specific target in the immune system. “When given to patients, this antibody blocks the action of two proteins (interleukin 12 and 23) that contribute to the inflammation and the overproduction of skin cells. By targeting these proteins, ustekinumab can interrupt the inflammatory pathway,” McCord says.

Antifungal agents for flexural psoriasis (to counteract Candida albicans). When we performed subgroup analyses, we found that preferences are, in part, dependent on sociodemographic and socioeconomic characteristics. Older patients ( ≥65 years) appear to care less about the probability of improvement compared with younger individuals. In most other studies20,21 using time trade-off, willingness to pay, and visual analog scales to assess health state utilities in psoriasis, utilities were largely independent of age. However, psoriasis-related impairment may be perceived differently among different age groups. For example, it has been reported5,12,40,41 that older patients experience psoriasis more physically than mentally compared with younger patients. Correspondingly, younger patients with psoriasis were shown22 to have greater willingness to pay for social comfort and emotional health than were older patients. Our descriptive analyses also showed the tendencies that women care more about the probability of improvement than men and that singles place greater value on this attribute compared with individuals living with a partner (Figure 2A and C). Although differences did not achieve statistical significance in our sample, it seems likely that women and singles attach special importance to appealing appearance, not the least because of expectations from society and from potential partners.

JAMA Network When is Binge Eating a Disorder? A proposal to minimize the toxicity of some of these medicines has been commonly called “rotational” therapy. The idea is to change the anti-psoriasis drugs every six to 24 months in order to minimize the toxicity of one medication. Depending on the medications selected, this proposal can be an option. An exception to this proposal is the use of the newer biologic medications as described below. An individual who has been using strong topical steroids over large areas of their body for prolonged periods may benefit from stopping the steroids for a while and rotating onto a different therapy.

The mechanism of action is unknown, although UVB light reduces DNA synthesis and can induce mild systemic immunosuppression. In PUVA, oral methoxypsoralen, a photosensitizer, is followed by exposure to long-wave UVA light (330 to 360 nm). PUVA has an antiproliferative effect and also helps to normalize keratinocyte differentiation. Doses of light are started low and increased as tolerated. Severe burns can result if the dose of drug or UVA is too high.

A convenient method of categorizing these drugs is on the basis of their site of action:

The side effects of ciclosporin are numerous and include hypertension, nephrotoxicity, hypomagnesaemia, hyperkalaemia, hyperuricaemia, elevation of liver function tests, development of paraesthesias, and hypertrichosis. In transplant patients, this immunosuppressive drug is associated with an increase in infections and lymphoproliferative diseases. If given for extended periods of time, nephrotoxicity occurs in most patients; hence, guidelines for the treatment of psoriasis with ciclosporin in the USA have suggested that it should not be administered for more than one year.33 All of the safety and efficacy data on the use of ciclosporin for psoriasis are based on doses up to 5 mg/kg daily, but the US Food and Drug Administration lists 4 mg/kg daily in the package insert. Nevertheless, it is usual to use higher doses for more severely affected patients.

FDA has approved a new oral drug, apremilast (Otezla), to treat psoriasis and psoriatic arthritis, with an entirely novel mode of action (inhibition of an enzyme, phosphodiesterase 4) and does not require intensive laboratory monitoring.
Topical steroids are safe and relatively easy to use for plaque psoriasis, scalp psoriasis, flexural psoriasis, sebopsoriasis and psoriasis affecting the palms and soles. They are not very effective in nail psoriasis.
Popular news One of the most common treatment options, topicals, are usually the first medication your doctor will prescribe after you’ve been diagnosed with psoriasis. They come in different forms, like creams, ointments, gels, or foam. Topicals work on top of the skin, where the symptoms appear – not under the skin, where symptoms start. Applying topicals directly on plaques can help slow the growth of skin cells.
Salicylic acid. Available over-the-counter (nonprescription) and by prescription, salicylic acid promotes sloughing of dead skin cells and reduces scaling. Sometimes it’s combined with other medications, such as topical corticosteroids or coal tar, to increase its effectiveness. Denise Mann, MSFeb 27 Skin cancers occur when skin cells undergo malignant transformations and grow into tumors. The most common types of skin cancer, basal cell carcinoma and squamous cell carcinoma, are highly curable when they are diagnosed and treated early. Sun exposure, tanning beds, depressed immune system, radiation exposure, and certain viral infections are risk factors for skin cancer. Skin cancers are treated with surgery or radiation. The prognosis of nonmelanoma skin cancers is generally very good.

Psoriasis medication: Methotrexate In the past, doctors treated psoriasis using a “step-wise approach.” Patients with mild to moderate psoriasis would start with topical therapies and, if they did not respond well to that, move on to other treatments, such as systemic therapy or phototherapy. This approach called for treating people with moderate to severe psoriasis with phototherapy or traditional systemic therapies—drugs such as methotrexate and cyclosporine—before offering them biologic therapies (a type of treatment that works with your immune system).

On average, participants with an intermediate monthly household income between €1000 and €2000 found the duration of benefit to be more important compared with those with lower income ( < €1000 per month; β  =  0.26; P  =  .02, Table 4) and viewed the severity of AEs as less important (P  =  .02 in descriptive analyses, Figure 3A; β  =   −0.25; P  =  .03 in regression models, Table 4). Furthermore, participants with intermediate incomes ( €1000- €2000 per month) attached less importance to the treatment delivery method compared with participants with lower income ( β  =   −0.32; P  =  .005). Notably, the importance of treatment cost covered by the individual did not differ significantly between income groups (data not shown). When attribute importance was compared between employed and unemployed participants (Figure 3B), employed participants were significantly more concerned about the reversibility of AEs (P  =  .006; data not shown). Patients with psoriasis should avoid injury to skin, including sunburn and other physical trauma, as these areas may develop psoriasis. The appearance of psoriatic lesions in previously uninvolved areas after irritation or trauma is known as the Köbner phenomenon. Patients with psoriasis should also, when feasible, avoid drugs known to worsen the problem (eg, chloroquine, beta-blockers, aspirin or other NSAIDs). They should also avoid alcohol to excess. An association has been made between nonalcoholic fatty liver disease and moderate-to-severe psoriasis. What is related to treatment and what is related to psoriasis itself is still being studied. By targeting these proteins, ustekinumab can interrupt the inflammatory pathway,” McCord says.

Moreover, we found that participants were willing to trade an increased risk of AEs for increased probability and magnitude of therapeutic benefit. This finding is in contradiction to the previously reported work of Seston and colleagues,27 who showed that patients with psoriasis prioritized low risk of AEs (skin cancer and liver damage) and that they were willing to trade time to achieve moderate (50%) skin improvement to minimize the risk of these AEs. In the CA exercise performed by the Seston group, AEs were precisely named, which may have influenced the measured preferences. However, preferences to avoid AEs were traded against “time to achieve moderate improvement ” rather than against probability and magnitude of improvement, making it difficult to compare their results with our own.

Boards study tools Your Profile Plaque psoriasis signs and symptoms appear as red or pink small scaly bumps that merge into plaques of raised skin. Plaque psoriasis classically affects skin over the elbows, knees, and scalp and is often itchy. Although any area may be involved, plaque psoriasis tends to be more common at sites of friction, scratching, or abrasion. Sometimes pulling off one of these small dry white flakes of skin causes a tiny blood spot on the skin. This is a special diagnostic sign in psoriasis called the Auspitz sign.

The most effective wavelengths of UVB light used for the treatment of psoriasis fall in a very narrow range, 311–313 nm.16,17 This has led to the development of narrowband UVB phototherapy, which is more efficient than broadband phototherapy.16 In the few years that narrowband UVB phototherapy has been used, no increase in cutaneous malignancies has been reported. More experience will be needed to firmly establish the safety of narrowband UVB phototherapy. The excimer laser is a powerful beam of 308 nm light (another form of narrowband ultraviolet light) that has been used successfully to treat localised plaques of psoriasis including those on the palms and soles.18
