Global Medical Knowledge Methotrexate (MTX), cyclosporine, and acitretin are the most commonly prescribed systemic medications for severe psoriasis in the United States.5 Despite the risk for hepatotoxicity, MTX appears to have the best combined safety and efficacy profile in terms of serious adverse events compared to other systemic agents.11 Guidelines for MTX monitoring, especially with regard to when to do a liver biopsy, have been substantially liberalized over time, and the recommended interval for biopsy has been extended by years; biopsy was previously recommended after a cumulative MTX dose of 1 to 1.5 g, but guidelines now suggest biopsy after 3.5 to 4 g in low-risk patients.5 While abnormally elevated liver function tests during treatment with MTX may necessitate liver biopsy, the use of transient elastography and a panel of serum biomarkers for liver function also can be used to monitor noninvasively for hepatotoxicity before biopsy is considered; these recommendations are likely to be incorporated into newer guidelines in development.12 Methotrexate has demonstrated safety and increased efficacy when used in combination with biologic agents such as adalimumab, etanercept, infliximab, and secukinumab7 and has been studied in combination with many biologics indicated for PsA.13

Psoralen is a photosensitizer that is ingested prior to light exposure. PUVA treatment results in conjunctival hyperemia and dry eye, particularly if sun protection is not used. With proper eye protection, there does not appear to be a risk of cataract. Psoralens for either topical (bath) or systemic use may occasionally be difficult to obtain because of intermittent availability issues.
Take daily baths. Bathing daily helps remove scales and calm inflamed skin. Add bath oil, colloidal oatmeal, Epsom salts or Dead Sea salts to the water and soak. Avoid hot water and harsh soaps, which can worsen symptoms; use lukewarm water and mild soaps that have added oils and fats. Soak about 10 minutes then gently pat dry skin.

One avenue of current research is looking at the skin and gut microbiome (the bacteria living on and in the human body) and whether the alteration of this microbiome may be effective in the treatment of psoriasis [4,5]. To date, probiotics have not been found to help psoriasis.

Calcipotriene cream is useful in psoriasis because of its effect on calcium metabolism. The advantage of calcitriol is that it is not known to thin the skin like topical steroids. There is a newer combination preparation of calcipotriene and a topical steroid called Taclonex. Not all patients may respond to calcipotriene. Prolonged use of these types of medications on more than 20% of the skin surface can produce an abnormal rise in body calcium levels.

Psoriasis is characterized by red, thickened plaques with a silvery scale. The lesions vary in size and degree of inflammation. Psoriasis is categorized as localized or generalized, based on the severity of the disease and its overall impact on the patient’s quality of life and well-being. Patient education about the disease and the treatment options is important. Medical treatment for localized psoriasis begins with a combination of topical corticosteroids and coal tar or calcipotriene. For lesions that are difficult to control with initial therapy, anthralin or tazarotene may be tried. The primary goal of therapy is to maintain control of the lesions. Cure is seldom achieved. If control becomes difficult or if psoriasis is generalized, the patient may benefit from phototherapy, systemic therapy and referral to a physician who specializes in the treatment of psoriasis.

Nevertheless, it is usual to use higher doses for more severely affected patients.

Remarkably, and in accordance with previous willingness-to-pay analyses in psoriasis,21,22 income was not a predictor for the effect of individual cost on preferences. Topical corticosteroids remain the most widely prescribed medications for plaque psoriasis. The Stoughton–Cornell classification ranks the potency of topical corticosteroids on their ability to induce vasoconstriction.3 Topical corticosteroids are available in numerous vehicles including powders, sprays, lotions, solutions, creams, emollient creams, ointments, gels, and tape. Recently, clobetasol propionate and betamethasone valerate have both been introduced in foam vehicles that are cosmetically elegant and should improve compliance. Different vehicles are used on different body sites. For example, the scalp and other hair bearing areas are most easily treated with foams, solutions, and gels. Creams are most useful for daytime use, and ointments, which are often more effective but less appealing cosmetically, can be applied at night. Two possible exceptions are the newer foam vehicles, which have comparable clinical efficacy to ointments.4,5

Although some basic treatment algorithms exist, patient preference, disease severity, and other variables including comorbidities (eg, psoriatic arthritis [PsA], risk of major cardiac events, inflammatory bowel disease [IBD]), history of nonmelanoma skin cancer (NMSC), pregnancy and lactation, and specific contraindications to therapy (eg, renal failure, liver disease, active malignancy) should be considered. In this article, we summarize common themes across existing guidelines and consensus statements for the treatment of psoriasis and highlight areas where there is consistent agreement or lack of sufficient information.

Choice of specific agents and combinations requires close cooperation with the patient, always keeping in mind the untoward effects of the treatments. There is no single ideal combination or sequence of agents, but treatment should be kept as simple as possible. Monotherapy is preferred, but combination therapy is the norm. At least 50 percent of all people with psoriasis have scalp psoriasis that can run the gamut from mild to severe. Although scalp psoriasis affects just two to three percent of the body, it has a major impact on our patients’ quality of life. Publish date: July 16, 2018 In all cases, the therapeutic goal is to maximize treatment efficacy and the patient’s quality of life, while minimizing side effects. Achieving and maintaining control of psoriatic lesions is the central goal in treatment. Physicians and patients need to understand that there is no definitive cure for psoriasis (Figure 4). Control of the disease may mean that lesions are not as thick or as red as they were before treatment, but some degree of erythema may remain. Most often, treatment does not result in complete clearing of the lesions.
