Although corticosteroids are rapidly effective in the treatment of psoriasis, they are associated with a rapid flare-up of disease after discontinuation, and they have many potential side effects. Consequently, topical corticosteroids are frequently used in conjunction with another agent to maintain control. Topical calcipotriene is often used in combination with topical corticosteroids to speed clearing of the lesions and maintain control after the initial phase of treatment is completed.

After providing written informed consent, participants were assigned an identification code used to access a computerized survey to be completed before their clinical consultation. Technical assistance was provided if requested. Within the survey, participants’ preferences were explored using CA. Generation and evaluation of the CA exercises involved (1) identification of key attributes associated with the range of psoriasis treatments; (2) assignment of various levels to the identified treatment attributes; (3) creation of hypothetical treatment scenarios by combining these levels in a random fashion; (4) pairing of the treatment scenarios using an orthogonal design to maximize discrimination of the relative importance of attributes while minimizing the number of choice tasks needed in the set; (5) presentation of choice sets to respondents; and (6) measurement of preferences for the treatment attributes based on the choices made for the presented choice sets.28 The range of currently available treatments was grouped into 6 outcome attributes (probability of benefit, magnitude of benefit, duration of benefit, probability of AEs, AE severity, and AE reversibility) and 5 process attributes (treatment location, frequency, duration, delivery method, and cost for the individual). Four realistic attribute levels, based on currently available treatments, were created for each attribute (Table 1).

Medical treatment for localized psoriasis begins with a combination of topical corticosteroids and coal tar or calcipotriene. For lesions that are difficult to control with initial therapy, anthralin or tazarotene may be tried. The primary goal of therapy is to maintain control of the lesions. Cure is seldom achieved. If control becomes difficult or if psoriasis is generalized, the patient may benefit from phototherapy, systemic therapy and referral to a physician who specializes in the treatment of psoriasis.

There is often some trial and error involved in reining in the itch and inflammation of scalp psoriasis. Certain over-the-counter (OTC) products and shampoos that contain tar or salicylic acid can help slow skin hyperkeratinization and reduce inflammation, itching, and scaling. If these OTC topical products aren’t effective, a dermatologist may prescribe Dovonex Cream (calcipotriene 0.005%, Leo), Taclonex Ointment (calcipotriene 0.005% and betamethasone dipropionate 0.064%, Leo) , Tazorac Cream or Gel (tazarotene 0.1%, Allergan), or Enstilar Foam (calcipotriene and betamethasone dipropionate, Leo). Tazarotene gel, a recently developed topical retinoid for psoriasis, is available in 0.05% and 0.1% gels and creams. Topical retinoids may reverse some of the cutaneous atrophy caused by topical corticosteroids12 but are associated with local cutaneous irritation. Thus, they are often prescribed in combination with topical corticosteroids.13
Expose your skin to small amounts of sunlight. A controlled amount of sunlight can improve psoriasis, but too much sun can trigger or worsen outbreaks and increase the risk of skin cancer. First ask your doctor about the best way to use natural sunlight to treat your skin. Log your time in the sun, and protect skin that isn’t affected by psoriasis with sunscreen.

Kimball, MD, MPH SITE MAP Psoriasis is a chronic autoinflammatory disorder affecting approximately 2% to 4% of the Western population.1 While there is no absolute cure for psoriasis, novel therapies allow for substantial reduction in symptoms and considerable improvement in quality of life (QoL). In the past few years, multiple treatment guidelines (recommendations based on evidence-based literature reviews) and consensus statements (a set of declarations determined and voted on by a panel of experts in the field) have been developed to guide physicians worldwide in treating psoriasis in the clinical setting (eTable).2-10

Chronic administration of methotrexate has been associated with the development of hepatic fibrosis. For a number of reasons documented in the literature, the frequency of hepatic fibrosis appears to be increased in patients with psoriasis compared to patients with rheumatoid arthritis. Consequently, guidelines call for periodic liver biopsies to monitor patients with psoriasis treated long term with methotrexate. Liver biopsies are recommended after 1.5 g cumulative methotrexate dose and approximately every 1.5 g thereafter if liver function tests are normal.29

(5) Wild-caught fish — Fish like salmon, mackerel, herring and sardines are excellent sources of vitamin D as well as omega-3 fatty acids, which are both key to improving psoriasis. Vitamin D is clinically proven to help fight psoriasis. (15) If you have psoriasis, fish should be the new leading protein in your life rather than meat and conventional dairy products. Studies have shown that eating fewer protein-rich foods, primarily meats and dairy products, may help ease psoriasis flare-ups.
ELIDEL Coal-tar ointments and shampoos. These products can help slow the rapid growth of skin cells and alleviate symptoms, but some people are vulnerable to the side effects, especially folliculitis, a pimple-like rash affecting the hair follicles. These medicines should be used only under a doctor’s supervision.
Low-potency corticosteroids (classes 6 and 7), such as desonide (Desowen), aclometasone dipropionate (Aclovate); hydrocortisone agents (Cortizone, Cortaid, etc.)
Salicylic acid. Available over-the-counter (nonprescription) and by prescription, salicylic acid promotes sloughing of dead skin cells and reduces scaling. Sometimes it’s combined with other medications, such as topical corticosteroids or coal tar, to increase its effectiveness. Salicylic acid is available in medicated shampoos and scalp solutions to treat scalp psoriasis.

Some doctors may prescribe ultraviolet B light (UVB) treatment using a light box alone or with other therapies such as coal tar. A more targeted ultraviolet light treatment, called narrow-band UVB therapy, is less carcinogenic than PUVA and almost as effective.
Several treatments may need to be tried before the most suitable regime is established, and different treatments may need to be used concurrently, or in rotation, for best effect or to minimise side-effects.
Understanding Plaque Psoriasis Recent Press Several forms of light therapy have been used to treat psoriasis for hundreds of years. In the 1920s, William Goeckerman combined the use of ultraviolet B (UVB) phototherapy with topical application of tars.14 This inpatient psoriasis regimen, known as the Goeckerman regimen, is still occasionally used, but outpatient regimens using UVB phototherapy with emollients have largely replaced the inpatient regimens.

calcipotriene Salicylic Acid This medicine is a keratolytic (peeling agent) that causes shedding of the outer layer of your skin. Besides psoriasis, it is used to treat acne, dandruff, and seborrhea, and to remove corns, calluses, and warts. There are many forms and brands of topical salicylic acid on the market. Salicylic acid topical is available as an ointment, liquid, gel, soap or shampoo, cloth pads, sprays, and skin patches. (8)
There are even more treatments for psoriasis on the horizon: IDP-118 (halobetasol propionate and tazarotene) lotion is an investigational topical treatment for plaque psoriasis. Halobetasol propionate is a steroid that cools inflammation, and tazarotene is a topical retinoid that slows the rate at which skin cells develop. Both ingredients are approved to treat plaque psoriasis, but due to side effects, their use is limited to shorter durations. The new formulation may cut back on those side effects allowing people to use the cream for longer periods of time. Stay tuned.
