The Lancet Oncology Chief Editor Allergist One of the most serious complications, which can accompany neurodermatitis, is Kaposi’s varicelliform eruption, due to which lethality ranges from 1.6 to 30% among children. The causative agent is the herpes simplex virus, especially of type 1, which causes damage to the upper respiratory tract and skin around the nose and mouth. Virus of the type 2 is less common, it affects mucous membranes and skin of genital organs. The disease begins acutely in 5-7 days after the contact with an ill person with herpes simplex and is manifested by chills, body temperature up to 40°C, weakness, adynamics, prostration. After 1-3 days rashes of small, to the size of a pinhead, bubbles, filled with serous, less commonly with hemorrhagic content appear. Later bubbles turn into pustules and acquire a typical appearance from navel like impression in the center. In the evolution of the elements bleeding erosions are formed, their surface is covered with hemorrhagic crusts. Patient’s face acquires “mask-like” appearance. The lesion of mucous membranes proceeds as aphthous stomatitis, conjunctivitis, and keratoconjunctivitis.

Age, education, occupation, number of children, contraceptive methods, disease duration, and dermatologic examination findings were recorded. All lesions in the patients were localized on their backs, where neurodermatitis lesions are commonly seen. We wanted to eliminate the localization factor from sexual function and evaluate the impacts of this skin disease alone on sexual function. Therefore, patients with anogenital localization were excluded from this study. Routine blood count, biochemistry, erythrocyte sedimentation rate, and urinalysis were performed for each participant. The functional integrity of the hypothalamo‐pituitary‐gonadal axis was examined in all participants by measuring the levels of follicle‐stimulating hormone, luteinizing hormone, estradiol, free testosterone, prolactin, and sex hormone–binding globulin. Participants suffering from pulmonary, hepatic, hematologic, renal, and endocrine diseases such as diabetes mellitus, as well as thyroid function disorders and inflammatory diseases such as rheumatoid arthritis, were excluded from the study. Other exclusion criteria included pregnancy and gynecologic and systemic disorders that might affect female sexual function such as hormonal, muscular, neurologic, or cardiovascular diseases and psychologic disorders. Participants on antidepressants and other drugs that are known to interfere with sexual function and postmenopausal women receiving oral or vaginal estrogen therapy for the prevention of climacteric symptoms or vaginal dryness were also excluded from the study.

The impact of neurodermatits and psoriasisi on sexual function had been studied previously. Inconsistent with our data, Mercan S found that neurodermatitis patients had more sexual problems than the psoriasis counterpart12. The difference may be due to the sample size. There were only 31 patients with neurodermatitis and 24 patients with psoriasis, while the counterpart in our study are 149 and 246 respectively. Another comment is related to the questionnaire applied in the survey. DLQI was applied in our study while Arizona Sexual Experience Scale in Mercan’s study. Although DLQI has question(Q9) about sexual life, it do not specifically evaluate sexual problems. Gender-specific sexual function scales and proper sample size will be needed in future study.

[61] Emerging clinical trials data indicate that blocking this pathway may be a key mechanism by which atopic itch can be treated clinically. Additionally, a 2017 study identified that neuronal, rather than immune, signaling of the type 2 cytokines IL-4 and IL-13 critically regulate AD-associated itch. [62] Indeed, the dual IL-4 and IL-13 blocker, dupilumab, has emerged as a highly effective treatment for AD, which received FDA approval in March of 2017. Thus, blocking cytokine-nerve interactions with targeted biologic therapies has emerged as a novel therapeutic strategy in AD.

Neurodermatitis is a skin condition that’s also known as lichen simplex chronicus. It causes patches of itchy skin that lead to scratching, which makes you feel even itchier. The condition isn’t contagious. It may be due to something that irritates your skin, like a bug bite or tight clothes that trigger the itch-scratch cycle.

Sitemap Author Footnotes That itching feeling that is normal for dry patches of skin may have no effect on some people and a chronic effect on others. The incidence of neurodermatitis, which is also known as atopic dermatitis or atopic eczema, and of psoriasis have increased in the last ten to fifteen years. Neurodermatitis is characterized by chronic itching or scaling of the skin. It starts with a patch of itchy skin, which leads to scratching. Scratching makes the affected area even itchier and the sufferer scratches a little more until he or she scratches out of habit. Chronic itching and scratching can cause the affected area to become thick and leathery. It is therefore essential to break this itch-scratch cycle.

Nummular Eczema Neurodermatitis: Treatment Lichen simplex chronicus (also called “neurodermatitis”) is a chronic eczematous eruption of the skin that is the result of scratching (Fig. 8.13). Pruritus precedes the scratching and may be precipitated by frustration, depression, and stress. The scratching then causes the lichenification and further itching, resulting in an “itch–scratch–itch” cycle that perpetuates the process.

Seborrheic dermatitis signs and symptoms may include:
Seborrheic dermatitis is a superficial fungal disease of the skin, occurring in areas rich in sebaceous glands. It is thought that an association exists between Malassezia yeasts and seborrheic dermatitis. This may, in part, be due to an abnormal or inflammatory immune response to these yeasts. The azoles represent the largest class of antifungals used in the treatment of this disease to date. In addition to their antifungal properties, some azoles, including bifonazole, itraconazole, and ketoconazole, have demonstrated anti-inflammatory activity, which may be beneficial in alleviating symptoms. Other topical antifungal agents, such as the allylamines (terbinafine), benzylamines (butenafine), hydroxypyridones (ciclopirox), and immunomodulators (pimecrolimus and tacrolimus), have also been effective. In addition, recent studies have revealed that tea tree oil (Melaleuca oil), honey, and cinnamic acid have antifungal activity against Malassezia species, which may be of benefit in the treatment of seborrheic dermatitis. In cases where seborrheic dermatitis is widespread, the use of an oral therapy, such as ketoconazole, itraconazole, and terbinafine, may be preferred. Essentially, antifungal therapy reduces the number of yeasts on the skin, leading to an improvement in seborrheic dermatitis. With a wide availability of preparations, including creams, shampoos, and oral formulations, antifungal agents are safe and effective in the treatment of seborrheic dermatitis.

Bengtsson A Basic Derm Curriculum Neurodermatitis is characterized by decreased serum levels of prostaglandin precursors, decrease of cAMP levels in leukocytes due to the defect of beta-adrenergic receptors, and also as a result of an increased activity of phosphodiesterase. It is believed that the consequence of cAMP decreased level can be an increased release of inflammatory mediators from leukocytes, including histamine, which causes decrease in the functional activity of T-lymphocytes via H2-receptor. This may explain IgE hyperproduction. The association with certain antibodies of tissue histocompatibility (HLA-A1, A9, B12, D24, DR1, DR7 and others) is found. According to the data of P. M. Alyeva (1993) DR5 antigen is a risk factor of disease development, and DR4 and DRw6 antigens are resistance factors. Most authors consider Vidal’s disease and disseminated neurodermatitis as an independent disease, but in patients with Vidal’s disease, the detection of immune phenomena, which are peculiar to atopic dermatitis, difference absence in the distribution of histocompatibility antigens in patients with the various prevalence of the process, the similarity in the disturbance of the metabolism of biogenic amines allow to consider disseminated neurodermatitis and Vidal’s disease as manifestations of one disease.

In the pathogenesis of neurodermatitis as well as eczema, the leading role belongs to a dysfunction of immune, central and autonomic nervous systems. The basis of immunological disorders is a reduction of a number and functional activity of T-lymphocytes, especially T-suppressors, regulating immunoglobulin E synthesis by B-lymphocytes. IgE binds to blood basophiles and mast cells, which start to produce histamine, causing immediate hypersensitivity development.

Share In healthy individuals, balance exists between important subsets of T cells (eg, Th1, Th2, Th17, Th22). The primary immune dysfunction hypothesis invokes an imbalance in the T cell subsets, with Th2 cells predominating; this results in the production of type 2 cytokines such as interleukin (IL)–4, IL-5, and IL-13, causing an increase in IgE from plasma cells. Later, in persons with chronic AD, the Th1 cells have been shown to predominate. More recently, Th17 cells have been found to be elevated in patients with AD. [9] Although primarily considered a Th2 cell‒associated cytokine-mediated disease, the precise contributions of Th1 and Th17 cell responses remain to be fully defined.

Background: Neurodermatitis is a common chronic skin disease. Although not life-threatening, it can produce an important psychosocial burden, sleep disturbance and sexual dysfunction. Patients with neurodermatitis tend to have poor social skills or interpersonal resources and a lack of flexibility. However quality of life (QoL) of patients with neurodermatitis has seldom investigated. The objective of this study is to assess the impact of neurodermatitis on patients’ QoL using the Dermatology Life Quality Index questionnaire, and assess its feasibility and internal consistency.

Good. Thank you for that information. Am I at risk of getting neurodermatitis?
Neurodermatitis is characterized by decreased serum levels of prostaglandin precursors, decrease of cAMP levels in leukocytes due to the defect of beta-adrenergic receptors, and also as a result of an increased activity of phosphodiesterase.

Vidal’s disease have a more favorable prognosis than disseminated neurodermatitis, although in the presence of the latter one, in the majority of patients the process regresses with the age, sometimes staying in the form of focal manifestations like eczema. Some authors suggest a possible link of the disease with Sezary syndrome.

Dog allergies are often caused by the allergens found in pollen, animal dander, plants, and insects, but dogs can also be allergic to food and medication as well. These allergies can cause symptoms such as excessive itching, scratching, and grooming; rashes; sneezing; watery eyes; paw chewing; and skin inflammation. In some cases, dogs have conditions, such as atopic dermatitis, that is associated with and may be caused by allergies.

The addition of the soak and smear method (soak in a warm water tub with no soap for 15-20 minutes; while still wet, apply the topical steroid, then dress in cotton pajamas and/or a vinyl sauna suit, preferably worn over damp cotton pajamas) will often speed improvement, as well as reduce pruritus dramatically. Patients must be cautious not to employ this modality for longer than 1-2 weeks to prevent side effects.

Network adequacy  There are a variety of treatments of Seborrheic dermatitis. Eradicating the Malasezia yeast appears to be important in the treatment. For this reason, I generally recommend use of various anti-dandruff shampoos including those containing zinc pyrithione, selenium sulphide, ketoconazole, and ciclopirox. These should be left on the scalp for 1-5 minutes depending on the specific patient’s scalp. Anti-dandruff shampoos can be drying, and so careful monitoring is needed to determine how best to use these for any given patent.
Native American Health Service Resident Rotation Seborrheic dermatitis is a superficial fungal disease of the skin, occurring in areas rich in sebaceous glands.

Seborrheic dermatitis is associated with normal levels of Malassezia but an abnormal immune response. Helper T cells, phytohemagglutinin and concanavalin stimulation, and antibody titers are depressed compared with those of control subjects. Zani et al report that even with antifungal treatments, there was no reduction in levels of Malassezia. [23] The contribution of Malassezia species to seborrheic dermatitis may come from its lipase activity—releasing inflammatory free fatty acids—and from its ability to activate the alternative complement pathway. Neurodermatitis is the multifactorial disease, in the development of which functional disorders of nervous system, immune disorders, and allergic reactions, as well as genetic predisposition have the greatest importance.
The term ‘prurigo nodularis’ (see p. 670) is used for lesions with a nodular clinical appearance and prominent epidermal hyperplasia of pseudoepitheliomatous rather than psoriasiform type. At times, lesions with overlapping clinical and histopathological features of both lichen simplex chronicus and prurigo nodularis are found. Prurigo nodularis is one condition in which the histological picture often lags behind the clinical appearance. The author has seen numerous cases, regarded by experienced dermatologists as prurigo nodularis, in which the histological picture was that of lichen simplex chronicus.

Symptoms can persist or begin in adulthood. It is also the most common cause of occupational skin disease in adults. This disease results from an interaction between susceptibility genes, the host's environment, pharmacological abnormalities, skin barrier defects, and immunological factors. New management approaches have evolved from advances in our understanding of the pathobiology of this common skin disorder. There is also an increase in mitochondrial enzymes in keratinocytes and in the number of melanocytes in the basal layer. Although these kinetic aspects are known, there is still no explanation for the pathogenesis of these plaques and the underlying pruritus; it is apparent, however, that self-induced trauma plays an important localizing role. The 2 types of contact dermatitis are irritant contact dermatitis (ICD) and allergic contact dermatitis (ACD).7-10 ICD, which accounts for approximately 80% to 90% of cases, is an inflammatory reaction of the skin typically resembling a burn that is caused by exposure to an irritant such as a chemical or household cleaning product.7-10 CD is commonly related to one’s occupation and tends to occur as a result of frequent and repeated use of hand hygiene products, exposure to chemicals, or glove use.7-10 Most ICD cases occur on exposed or unprotected areas of the skin, such as the face, hands, and arms.7-10 Approximately 80% of ICD cases occur on the hands and another 10% involve areas of the face.7-10 The most common symptoms include inflammation, edema, erythema, and crusting and formation of pustules or vesicles.7-10
