Anxiety and stress can trigger the itch associated with neurodermatitis. Atopic dermatitis (AD) (or atopic eczema) is a chronic skin condition that affects the quality of life of both adults and children. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety. Diseases As sweating makes itching worse, it helps not to have too many bed covers on at night and, during the day, to wear clothing appropriate to the temperature. Twitter News & Perspective Drugs & Diseases CME & Education Academy Video New Allergy. 2000; 55: 551-555 AAFA The earliest descriptions of AD have referred to it as “neurodermatitis” due to the belief that the itch and scratch cycle, which results in a rash, was related to nerves and emotions. Though a lot more has been learned about immunopathogenesis of AD, recognition of itching and scratching as hallmarks of this disease remains true. Itching is often worse at night. Chronic sleep disturbance due to frequent scratching can have adverse effects on the patient and his/her immediate family members and is a source of significant stress. When subjects with AD get upset, they tend to itch even more, probably secondary to flushing of the skin due to vasodilatation induced by neurogenic peptides, followed by histamine and prostaglandin E2 (Ostlere, Cowen et al. 1995). Papules, lichenification, and eczematous lesions result from repeated skin trauma. Vesicles and erythema with excoriation of the skin are seen in acute eczema. Crusted lesions and pustules are seen in infected eczema. Allergens, irritants (wool, soap, detergents, heat and humidity with sweating), infections, and certain foods can worsen eczema, and such history helps establish the right diagnosis. Distribution of rash on the face and extensor surfaces in infants and young children, changing to mostly flexural involvement in older ages, is a classic finding of AD and one of the essential features required to make this diagnosis. Dry skin (xerosis), personal and family history of allergic diseases, palmar hyperlinearity, nipple eczema, kera-tosis pilaris, pityriasis alba, and non-specific hand or foot dermatitis are some of the other clinical manifestations of this condition (Leung and Bieber 2003; Leung, Jain et al. 2003). The impact of AD on self-esteem and social interactions of kids or adults with this condition cannot be underestimated (Lapidus and Kerr 2001). Patients with neurodermatitis scored significantly lower for all items except Q1 (symptoms) and Q9 (sexual difficulties). No strong relationship between disease-related characteristics and quality of life could be found. The inter-item correlation averaged 0.415 and Cronbach's alpha was 0.889, indicating high internal consistency. Schlievert PM Alumni Center In the adult population, the rate of AD frequency is 3% or higher, but onset may be delayed until adulthood. Covering the affected area can help the medicine penetrate thickened skin and prevent scratching. Other ways to avoiding scratching include cutting fingernails very short or using coal tar preparation. Neurodermatitis is characterized by marked uniform acanthosis with an elongation of epithelial processes; spongiosis without the formation of bubbles: a granular layer is weakly expressed or absent, hyperkeratosis, sometimes alternating with parakeratosis. In the dermis there is a mild perivascular infiltrate. The aha! Swiss Allergy Centre uses cookies on its www.aha.ch website to ensure you get the best experience. Niemeier et al (1997) compared sexual behaviors in patients with psoriasis, neurodermatitis, and controls. Patients with skin diseases had a significantly impaired sexual life, compared with healthy controls (Niemeier et al, 1997). Gupta and Gupta (1997) reported that 40.8% of patients with psoriasis expressed that their sexual activity had diminished since the onset of their skin disease. Sampogna et al (2007) investigated the sexual life in patients with psoriasis with 2 dermatology‐specific questionnaires (Skindex‐29 and DLQI) and 2 psoriasis‐specific questionnaires (Psoriasis Disability Index [PDI] and Impact of Psoriasis on Quality of Life Questionnaire [IPSO]). Of 936 patients, 35.5% (PDI) to 71.3% (IPSO) reported sexual problems because of psoriasis. Mercan et al (2008) used the Arizona Sexual Experience Scale (ASEX) to compare sexual function in 31 patients with neurodermatitis, 24 patients with psoriasis, and 33 control cases. They found that the neurodermatitis group had more sexual problems than the psoriasis group and the control group. Sukan and Maner (2007) used the ASEX to examine sexual problems in 50 patients with vitiligo, 50 patients with chronic urticaria, and 50 healthy controls. The total scores of the ASEX in the female patient groups were significantly higher than those in controls. The score for satisfaction from orgasm in male patients was less than that in controls. In men, differences in other subitem scores of ASEX were not significant between the patient groups and controls. The structure of normal skin and the morphology of atopic eczema. Atopic dermatitis (AD) is associated with colonization and increased risk of infections with Staphylococcus aureus. However, it's unclear how the skin microbiome during infancy affects the incidence of AD. This prospective study evaluated the effects of skin microbiome on the risk of developing AD during the first year of life. The study included 50 randomly selected infants from an Irish birth cohort study. Skin swabs from four sites relevant to AD were collected at 2 days and 2 and 6 months of age, with bacterial 16S rRNA gene sequencing and analysis performed directly from clinical samples. Patterns and changes in bacterial skin colonization were analyzed for association with the incidence of AD at 1 year. The types and diversity of bacteria in the skin microbiome changed significantly between sampling periods. In contrast to patients with established AD, occurrence of infantile AD was not associated with skin dysbiosis or colonization with S. aureus. Development of AD during the first year of life was associated with significant differences in bacterial communities detected in swabs from the antecubital fossa at 2 months. In particular, commensal staphylococci were significantly less abundant in infants who went on to develop AD. The skin microbiome was unrelated to mode of delivery or feeding method. Skin colonization with S. aureus does not appear to occur before the development of AD in infants. Early colonization with commensal staphylococci may have a protective effect against infantile AD. The authors emphasize the need for further research to understand the pathophysiology and mechanisms by which the skin microbiome affects the development of skin immunity and AD.
