Mycophenolate mofetil is effective in the treatment of atopic dermatitis. Have you been stressed or anxious lately? Leadership Another cause of Kaposi varicelliform eruption is vaccination with vaccinia for the prevention of small pox, but because this is no longer mandatory, patients with AD do not develop the sequelae of eczema vaccinatum that has been seen in the past. It was usually contracted by the patient from the vaccination of themselves or their close relatives. This condition had a high mortality rate (up to 25%). In the current climate of threats of bioterrorism, vaccination may once again become necessary, and physicians should be aware of eczema vaccinatum in this setting. Cochrane Scholarship Patients with atopic dermatitis need safe and effective long-term treatments. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. Keller P Management Atopic dermatitis (AD) is a pruritic disease of unknown origin that usually starts in early infancy (an adult-onset variant is recognized); it is characterized by pruritus, eczematous lesions, xerosis (dry skin), and lichenification (thickening of the skin and an increase in skin markings). It was intended to refer to localized circumscribed patches of lichenified skin associated with itching. Without proper knowledge of its etiology or its appearance it can be confused with many other conditions, resulting in poor management. The authors characterize the diagnosis as it was intended and give a capsule description of its appearance, differential diagnosis, prognosis and management. Experiments in murine models and human cell culture suggest that Th2-mediated inflammation weakens the skin barrier and creates susceptibility to xerosis (ie, abnormal dryness of the skin) and highly pruritic lesion formation.12,13 Feeling especially itchy when life becomes stressful Pregnancy YouTube Immune deficiency diseases Ankles For You Ferdi Ozturk FEATUREDKidney Diets for Cats: What to Look for The following is a constellation of symptoms and features commonly seen in AD: You are here Learning/CME Readers Position statements Government Chao-Kuei Juan, Hsuan-Ju Chen, Jui-Lung Shen, Chia-Hung Kao and Shang-Jyh Hwang, Lichen Simplex Chronicus Associated With Erectile Dysfunction: A Population-Based Retrospective Cohort Study, PLOS ONE, 10, 6, (e0128869), (2015). Fundraise Doherty DE Chronic Fatigue Syndrome – What do you know about CFS? New York Reptile & Amphibian Center The epidermal barrier dysfunction hypothesis suggests that AD patients develop AD as a result of skin barrier defects that allow for the entry of antigens, resulting in the production of inflammatory cytokines. Some authors question whether such antigens can also be absorbed from the gut (eg, from food) and/or the lungs (eg, from house dust mites). Xerosis and ichthyosis are known to be associated signs in many AD patients. Clinically, 37-50% of people with ichthyosis vulgaris have atopic disease and up to 37% of people with AD have clinical evidence of ichthyosis vulgaris. Mutations in the gene encoding filaggrin, a key epidermal barrier protein, cause ichthyosis vulgaris and are the strongest known genetic risk factors for the development of AD.
