Some patients with lichen simplex chronicus have a history of emotional or psychiatric problems. However, for most, it is simply a nervous habit. Preceding the eruption, the patient has a pruritic area of skin that is scratched, producing a plaque of chronic dermatitis. Epidemiology Keratosis pilaris Partner: ABSTRACT: Neurodermatitis is a chronic disease affecting the patient's psychosocial status and quality of life. It is associated with a variety of psychologic problems, including demoralization, depression, anxiety, obsessive‐compulsive disorder, and sleep disturbances. Coexistence of sexual dysfunction, especially in women, with several systemic diseases has gained interest in recent years. In this study, we evaluated sexual function in female patients with neurodermatitis. We enrolled 89 women (43 patients, 46 controls) in the study. Quality of life was assessed with the Dermatology Life Quality Index (DLQI), and the Female Sexual Function Index (FSFI) was used to determine sexual function. Individuals with psychiatric disorders and/or those using antidepressants were excluded. The total DLQI score was 11.95 ± 5.65 in patients with neurodermatitis. The total FSFI score was significantly lower in patients compared with healthy controls (22.76 ± 5.31 and 28.83 ± 3.50, respectively; P = .001). Domain scores of FSFI (desire, arousal, lubrication, orgasm, and satisfaction) except pain were significantly lower in patients with neurodermatitis (P = .001). The pain score was also lower in patients than controls, but the difference was not statistically significant (P = .073). Neurodermatitis may be associated with sexual dysfunction, and patients with neurodermatitis should be evaluated with regard to sexual function to provide a better quality of life. People should pay more attention to the chronic disease, although it is not life-threatening. There are two limitations in this study: we were unable to compare QoL of patients before and after treatment, which could be incorporated in the planning of future studies. Another comment is related to the questionnaire itself, part of patients worried about the impact of diet on disease which does not exist in DLQI because of the different culture background. Comments from Stan Fineman, MD: Skin barrier impairment in newborns is a potential risk factor for AD. Water can be a skin irritant and hard water, with high calcium carbonate, could cause even more irritation. This report from the Danish National Birth Cohort is unique since the geology of Denmark is unusual, with the country separated by a “main stationary line” caused by glaciers. The risk for developing AD increased by 5% for each 5 degrees of domestic water hardness, suggesting that using softer water could potentially be a primary prevention measure. Although the prevalence of AD increased in infants born in the colder seasons as expected, there was no synergism with hard water exposure. Maybe we should pay closer attention to the hardness of our local water. Changes like those of lichen simplex chronicus may be superimposed on other dermatoses such as lichen planus (hypertrophic lichen planus), mycosis fungoides, actinic reticuloid, and eczematous dermatitides including atopic dermatitis. These changes are particularly prominent in some solar keratoses of the hands or forearms.
