However, it's unclear how the skin microbiome during infancy affects the incidence of AD. This prospective study evaluated the effects of skin microbiome on the risk of developing AD during the first year of life. The study included 50 randomly selected infants from an Irish birth cohort study. Skin swabs from four sites relevant to AD were collected at 2 days and 2 and 6 months of age, with bacterial 16S rRNA gene sequencing and analysis performed directly from clinical samples. Patterns and changes in bacterial skin colonization were analyzed for association with the incidence of AD at 1 year. The types and diversity of bacteria in the skin microbiome changed significantly between sampling periods. In contrast to patients with established AD, occurrence of infantile AD was not associated with skin dysbiosis or colonization with S. aureus. Development of AD during the first year of life was associated with significant differences in bacterial communities detected in swabs from the antecubital fossa at 2 months. In particular, commensal staphylococci were significantly less abundant in infants who went on to develop AD. The skin microbiome was unrelated to mode of delivery or feeding method. Skin colonization with S. aureus does not appear to occur before the development of AD in infants. Early colonization with commensal staphylococci may have a protective effect against infantile AD. The authors emphasize the need for further research to understand the pathophysiology and mechanisms by which the skin microbiome affects the development of skin immunity and AD. The scree plot showed a sharp drop in eigenvalues from the first to the second component, with subsequent components extracting progressively less of the variance. This indicated that a one-dimensional solution is to be preferred (Fig. (Fig.1).1). The loadings of the DLQI items are given in Table Table6.6. The impact of neurodermatits and psoriasisi on sexual function had been studied previously. Inconsistent with our data, Mercan S found that neurodermatitis patients had more sexual problems than the psoriasis counterpart12. The difference may be due to the sample size. There were only 31 patients with neurodermatitis and 24 patients with psoriasis, while the counterpart in our study are 149 and 246 respectively. Another comment is related to the questionnaire applied in the survey. DLQI was applied in our study while Arizona Sexual Experience Scale in Mercan's study. Although DLQI has question(Q9) about sexual life, it do not specifically evaluate sexual problems. Gender-specific sexual function scales and proper sample size will be needed in future study. Understanding research results The term ‘prurigo nodularis’ (see p. 670) is used for lesions with a nodular clinical appearance and prominent epidermal hyperplasia of pseudoepitheliomatous rather than psoriasiform type. At times, lesions with overlapping clinical and histopathological features of both lichen simplex chronicus and prurigo nodularis are found. Prurigo nodularis is one condition in which the histological picture often lags behind the clinical appearance. The author has seen numerous cases, regarded by experienced dermatologists as prurigo nodularis, in which the histological picture was that of lichen simplex chronicus. It causes patches of itchy skin that lead to scratching, which makes you feel even itchier. The condition isn’t contagious. It may be due to something that irritates your skin, like a bug bite or tight clothes that trigger the itch-scratch cycle. Reich K AllergyWatch, Stanford, California Cross-reactions Monthly newsletter Neurodermatitis, also known as lichen simplex chronicus, is a common chronic skin disease, affecting up to 12% of the total population, and women are more affected than men1. The disease is characterized by lichenificated plaque as a result of excessive scratching. Neck, elbow, ankles, vulva, eyelid even faces are the most common affected sites. Although neurodermatitis is not life-threatening, it can produce an important psychosocial burden. It has been suggested that patients with neurodermatitis suffer from depression, anxiety and other treatable psychological disorders2. Negative emotional states are the main personality component of patients (greater tendency to pain avoidance, greater dependency on other peoples' desires, and more conforming and dutiful)3. Patients with neurodermatitis tend to have poor social skills or interpersonal resources and a lack of flexibility. Neurodermatitis may be associated with sleep disturbance and sexual dysfunction4. All these data constitute a growing body of evidence indicating a negative impact of neurodermatitis on patients' quality of life (QoL).
