History Scopus (86) Frequency Healthy Kids J Allergy Clin Immunol. 1992; 90: 184-193 Patients with atopic dermatitis need safe and effective long-term treatments. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. According to modern concepts, neurodermatitis is the genetically determined disease with multifactorial inheritance of predisposition to allergic reactions. Importance of genetic factors is confirmed by a high rate of disease among close relatives and monozygotic twins. According to an immunogenetic study, neurodermatitis is significantly associated with HLA B-12 and DR4. Lichen simplex chronicus is a chronic, waxing and waning problem that accompanies the mood changes of the patient. Cookies Donate to Wikipedia The diagnosis of lichen simplex chronicus is usually obvious in patients who have a localized itching plaque of chronic dermatitis. Chronic contact dermatitis should be considered from a topical medicine used to treat the symptoms. Several more serious “psychodermatoses” are habitual (neurotic) excoriations, factitious dermatitis, and delusions of parasitosis. This article is available to subscribers. The spring allergy season begins with pollens released by trees and then grasses follow later in spring into summer. There are apps you can use to watch your area’s pollen counts. On days that the pollens are high for the trees or grasses you are allergic to, you can take these actions to lessen the amount of pollens you are exposed to: Presence of IgE antibodies to staphylococcal exotoxins on the skin of patients with atopic dermatitis. Diseases of the ear, throat and nose (otolaryngology) Horse Stop the itch: This is the main goal of treatment because for treatment to be effective, you must stop scratching, rubbing, and touching the itchy patch. To stop the itch, your dermatologist may include one or more of the following in your treatment plan: Murine model of atopic dermatitis associated with food hypersensitivity. Hugo S Dyshidrotic Eczema Subscription Agents Comments by Stan Fineman, MD: This birth cohort study analyzed data from skin samples at age 2 days, 2 months, and 6 months, with gene sequencing to determine microbiome changes. Children were then followed up for the clinical development of AD. Interestingly the presence of S. aureus did not precede the development of AD—in fact, several commensal Staph species present at 2 months were associated with a reduced incidence of AD. This suggests that the relationship between the microbiome and skin inflammation is complicated, and that some commensal bacteria may even have a protective effect against eczema in infants.
