More recent information examining physician visits for AD in the United States from 1997-2004 estimates a large increase in office visits for AD occurred. In addition, blacks and Asians visit more frequently for AD than whites. Note that this increase involves all disease under the umbrella of AD and it has not been possible to allocate which type has increased so rapidly. The causative agent is the herpes simplex virus, especially of type 1, which causes damage to the upper respiratory tract and skin around the nose and mouth. Virus of the type 2 is less common, it affects mucous membranes and skin of genital organs. The disease begins acutely in 5-7 days after the contact with an ill person with herpes simplex and is manifested by chills, body temperature up to 40°C, weakness, adynamics, prostration. After 1-3 days rashes of small, to the size of a pinhead, bubbles, filled with serous, less commonly with hemorrhagic content appear. Later bubbles turn into pustules and acquire a typical appearance from navel like impression in the center. In the evolution of the elements bleeding erosions are formed, their surface is covered with hemorrhagic crusts. Patient's face acquires "mask-like" appearance. The lesion of mucous membranes proceeds as aphthous stomatitis, conjunctivitis, and keratoconjunctivitis. Common Emergencies Menopause 11. Szepietowski JC, Reich A, Wiśnicka B. NIH: National Institute of Arthritis and Musculoskeletal and Skin Diseases Lila and Murray Gruber Cancer Research Award and Lectureship Werfel T Nilsson EJ Eczema in Children The Mission Factors affecting skin barrier function might influence the risk of atopic dermatitis (AD). Previous reports have suggested higher rates of AD in areas with so-called hard water and among children born in fall or winter. This study looked for a possible synergistic relationship between these two AD risk factors. The researchers analyzed data on nearly 53,000 children from the Danish National Birth Cohort. National data sources were used to gather birth data and information on domestic water hardness, which differs across Danish regions. Physician diagnosis of AD and other characteristics were assessed in parental interviews when the children were 6 and 18 months old. Overall prevalence of AD was 15.0%. Prevalence ranged from 13.5% to 17.1% in regions with the softest versus hardest domestic water; relative prevalence of AD increased by 5% per 5-degree increase in water hardness. Hard water accounted for an estimated 2% of the population risk of AD. Relative prevalence of AD was 24% higher for children born in fall and 18% higher for those born in winter. There was no evidence of interaction between season of birth and water hardness. Both hard domestic water and fall/winter birth are associated with a higher prevalence of AD in early childhood. Risk of AD increases along with degree of water hardness, but there is no synergistic interaction between these two risk factors. Further studies are needed to see if water softening can reduce AD risk in infants. How to order Skip to main content The epidermal barrier dysfunction hypothesis suggests that AD patients develop AD as a result of skin barrier defects that allow for the entry of antigens, resulting in the production of inflammatory cytokines. Some authors question whether such antigens can also be absorbed from the gut (eg, from food) and/or the lungs (eg, from house dust mites). Xerosis and ichthyosis are known to be associated signs in many AD patients. Clinically, 37-50% of people with ichthyosis vulgaris have atopic disease and up to 37% of people with AD have clinical evidence of ichthyosis vulgaris. Mutations in the gene encoding filaggrin, a key epidermal barrier protein, cause ichthyosis vulgaris and are the strongest known genetic risk factors for the development of AD. A retrospective study of 23 exposed women revealed nothing notable (Franssen 1999). Experimentally, coal tar products have, to some extent, demonstrated mutagenic or carcinogenic properties, but there has not yet been any indication of this in the longstanding and well-tried use of the group of substances employed therapeutically in humans.
