This makes your skin more likely to become infected by bacteria or viruses. Featured Topics Patient advocates Support IgE receptors. Abeck D Bonnekoh B What is an allergy? For Developers In addition to the role of T and B cells in AD, other innate immune cells have also been implicated in the pathogenesis of AD, including eosinophils and mast cells. [10, 11] More recently, basophils and newly identified innate immune cells called group 2 innate lymphoid cells (ILC2s) have been shown to underlie the pathogenesis of AD. [12, 13, 14, 15, 16] Together, basophils and ILC2s are critical sources of the type 2 cytokines IL-4, IL-5, and IL-13. [12, 13] Further, these cells appear to be potently regulated by a family of epithelial cell‒derived cytokines directly released from damaged keratinocytes, including thymic stromal lymphopoietin (TSLP), IL-25, and IL-33. [17] Taken together, these studies highlight a new paradigm in which, in addition to classical adaptive Th2 cells, innate type 2 immune cells play critical roles in the etiology of AD through interactions with epidermal-derived cytokines. Virginia Atopic dermatitis (AD) (or atopic eczema) is a chronic skin condition that affects the quality of life of both adults and children. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety. Senior Health Tiếng Việt Professionalism Award Featured Topics The occurrence of gestational diabetes should be considered with tacrolimus therapy. In cases involving long-term prenatal therapy in the second and/or third trimesters, the newborn should be monitored for transient renal insufficiency and hyperkalemia. This applies to tacrolimus, everolimus, and sirolimus. There are insufficient data concerning the safety of first-trimester use of sirolimus, everolimus, and mycophenolate mofetil. From the 200 pregnancies exposed to tacrolimus no substantial teratogenic risk can be concluded. However, the use of these drugs does not require termination of pregnancy. A detailed fetal ultrasound examination should be offered to confirm normal morphologic development in cases of first-trimester exposure. Whether the immunosuppressant therapy of a transplanted woman who is stable on her medication should be changed is a question of risk–benefit assessment, because there is little experience in human pregnancies. ABSTRACT: Neurodermatitis is a chronic disease affecting the patient's psychosocial status and quality of life. It is associated with a variety of psychologic problems, including demoralization, depression, anxiety, obsessive‐compulsive disorder, and sleep disturbances. Coexistence of sexual dysfunction, especially in women, with several systemic diseases has gained interest in recent years. In this study, we evaluated sexual function in female patients with neurodermatitis. We enrolled 89 women (43 patients, 46 controls) in the study. Quality of life was assessed with the Dermatology Life Quality Index (DLQI), and the Female Sexual Function Index (FSFI) was used to determine sexual function. Individuals with psychiatric disorders and/or those using antidepressants were excluded. The total DLQI score was 11.95 ± 5.65 in patients with neurodermatitis. The total FSFI score was significantly lower in patients compared with healthy controls (22.76 ± 5.31 and 28.83 ± 3.50, respectively; P = .001). Domain scores of FSFI (desire, arousal, lubrication, orgasm, and satisfaction) except pain were significantly lower in patients with neurodermatitis (P = .001). The pain score was also lower in patients than controls, but the difference was not statistically significant (P = .073). Neurodermatitis may be associated with sexual dysfunction, and patients with neurodermatitis should be evaluated with regard to sexual function to provide a better quality of life. J Allergy Clin Immunol. 2000; 105: 782-787 Jiang Can and Bai Ji Li are able not only to expel external Wind, but also to extinguish internal Wind, the Liver-Wind. They are more suitable for use when the patient has Liver-Yin and Blood deficiency, and disturbance of the Liver-Wind, such as seen in pruritus in elderly people, skin diseases starting or worsening in stressful situations, itchy rashes in the evening, in the night or after menstruation, and chronic skin diseases. Since external Wind and internal Wind often coexist—for instance, the itch may not only be worse in the above conditions, but also when the weather changes, after taking spicy food or during cold infections—therefore substances that expel external Wind and those that extinguish internal Wind can be used together.
