Funding and Disclosures Medications Atopic dermatitis (AD) is associated with colonization and increased risk of infections with Staphylococcus aureus. However, it's unclear how the skin microbiome during infancy affects the incidence of AD. This prospective study evaluated the effects of skin microbiome on the risk of developing AD during the first year of life. The study included 50 randomly selected infants from an Irish birth cohort study. Skin swabs from four sites relevant to AD were collected at 2 days and 2 and 6 months of age, with bacterial 16S rRNA gene sequencing and analysis performed directly from clinical samples. Patterns and changes in bacterial skin colonization were analyzed for association with the incidence of AD at 1 year. The types and diversity of bacteria in the skin microbiome changed significantly between sampling periods. In contrast to patients with established AD, occurrence of infantile AD was not associated with skin dysbiosis or colonization with S. aureus. Development of AD during the first year of life was associated with significant differences in bacterial communities detected in swabs from the antecubital fossa at 2 months. In particular, commensal staphylococci were significantly less abundant in infants who went on to develop AD. The skin microbiome was unrelated to mode of delivery or feeding method. Skin colonization with S. aureus does not appear to occur before the development of AD in infants. Early colonization with commensal staphylococci may have a protective effect against infantile AD. The authors emphasize the need for further research to understand the pathophysiology and mechanisms by which the skin microbiome affects the development of skin immunity and AD. An obligate sign of the first period is the localization of lesions on the cheeks. Primary rashes are characterized by erythematous, edematous and erythematous squamous foci, papules, vesicles, madescence and crusts, so-called infantile eczema. Then the process gradually extended to the collar area (breastplate zone), upper extremities. At the 2nd year of life of a child exudative phenomena subside and give way to the emergence of small polygonal shiny papules, accompanying by itching. Furthermore, rashes tend to limitation and located in the talocrural, wrist, elbow and neck wrinkles. CT scan Scopus (125) In healthy individuals, balance exists between important subsets of T cells (eg, Th1, Th2, Th17, Th22). The primary immune dysfunction hypothesis invokes an imbalance in the T cell subsets, with Th2 cells predominating; this results in the production of type 2 cytokines such as interleukin (IL)–4, IL-5, and IL-13, causing an increase in IgE from plasma cells. Later, in persons with chronic AD, the Th1 cells have been shown to predominate. More recently, Th17 cells have been found to be elevated in patients with AD. [9] Although primarily considered a Th2 cell‒associated cytokine-mediated disease, the precise contributions of Th1 and Th17 cell responses remain to be fully defined. More scratching leads to more thickening, which may cause the thickening to spread into a leathery brown patch. Very thick skin can even have a grayish hue. On the other hand, there is one case report of multiple congenital anomalies in a fetus which was exposed to MMF, tacrolimus, and prednisone (le Ray 2004); another child of a liver transplanted mother was born with atresia of the esophagus, and malformations of the heart and iris (Källén 2005). The notion that stress contributes somehow to onset or progression of these disorders does not indicate how strong the influence of stress is compared to other important contributing factors, such as genetic predisposition or organic diseases.
