What prognosis does neurodermatitis have? Neurodermatitis is complicated by recurrent bacterial, viral and fungal infections, especially in those patients, who use hormonal ointments for a long time. Bacterial complications are represented by folliculitis, furunculosis, impetigo, and hydradenitis. The causative agent of these complications is usually Staphylococcus aureus, less epidermal one, white staphylococci or streptococci, the source of which are centers of chronic infection. The development of complications is accompanied by chill, increase of body temperature, sweating, increased congestion and itching. Peripheral lymph nodes are enlarged, painless. The occurrence of gestational diabetes should be considered with tacrolimus therapy. In cases involving long-term prenatal therapy in the second and/or third trimesters, the newborn should be monitored for transient renal insufficiency and hyperkalemia. This applies to tacrolimus, everolimus, and sirolimus. There are insufficient data concerning the safety of first-trimester use of sirolimus, everolimus, and mycophenolate mofetil. From the 200 pregnancies exposed to tacrolimus no substantial teratogenic risk can be concluded. However, the use of these drugs does not require termination of pregnancy. A detailed fetal ultrasound examination should be offered to confirm normal morphologic development in cases of first-trimester exposure. Whether the immunosuppressant therapy of a transplanted woman who is stable on her medication should be changed is a question of risk–benefit assessment, because there is little experience in human pregnancies. Two phase 3 trials of dupilumab versus placebo in atopic dermatitis. N Engl J Med. 2016;375:2335–2348. Ocular/periorbital changes Allergy card By the end of the second period, an "atopic face" develops hyperpigmentation and wrinkles in the area of eyelids, imparting the child “tired appearance”. The rest of skin is also changed, but without clinically pronounced inflammation (dryness, dullness, defurfuration, dyschromia, infiltration). Neurodermatitis is characterized by seasonality disease course and is in the development of exacerbations in autumn and winter and a significant improvement or resolution of the process in summer, especially in the conditions of south. Patients with atopic dermatitis need safe and effective long-term treatments. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. More recent information examining physician visits for AD in the United States from 1997-2004 estimates a large increase in office visits for AD occurred. In addition, blacks and Asians visit more frequently for AD than whites. Note that this increase involves all disease under the umbrella of AD and it has not been possible to allocate which type has increased so rapidly. It is associated with a variety of psychologic problems, including demoralization, depression, anxiety, obsessive‐compulsive disorder, and sleep disturbances. Coexistence of sexual dysfunction, especially in women, with several systemic diseases has gained interest in recent years. In this study, we evaluated sexual function in female patients with neurodermatitis. We enrolled 89 women (43 patients, 46 controls) in the study. Quality of life was assessed with the Dermatology Life Quality Index (DLQI), and the Female Sexual Function Index (FSFI) was used to determine sexual function. Individuals with psychiatric disorders and/or those using antidepressants were excluded. The total DLQI score was 11.95 ± 5.65 in patients with neurodermatitis. The total FSFI score was significantly lower in patients compared with healthy controls (22.76 ± 5.31 and 28.83 ± 3.50, respectively; P = .001). Domain scores of FSFI (desire, arousal, lubrication, orgasm, and satisfaction) except pain were significantly lower in patients with neurodermatitis (P = .001). The pain score was also lower in patients than controls, but the difference was not statistically significant (P = .073). Neurodermatitis may be associated with sexual dysfunction, and patients with neurodermatitis should be evaluated with regard to sexual function to provide a better quality of life. Amazon Devices Customer Support Bahasa Indonesia Psychiatry In the fresh foci disseminated neurodermatitis has acanthosis, edema of dermis, sometimes spongiosis and exocytosis, as in the presence of eczema. In the dermis perivascular infiltrates of lymphocytes with an admixture of neutrophils are present. In older foci, except acanthosis, hyperkeratosis and parakeratosis, sometimes, spongiosis are expressed. In the dermis dilatation of capillaries with endothelial swelling, around which small infiltrates of the lymphatic and histiocytic nature with a significant number of fibroblasts can be seen. In the central part of lesion focus pigment is not detected in the basal layer, whereas in peripheral parts of it, particularly in older foci of lichenification, melanin amount is increased.
