Typo comment The occurrence of gestational diabetes should be considered with tacrolimus therapy. In cases involving long-term prenatal therapy in the second and/or third trimesters, the newborn should be monitored for transient renal insufficiency and hyperkalemia. This applies to tacrolimus, everolimus, and sirolimus. There are insufficient data concerning the safety of first-trimester use of sirolimus, everolimus, and mycophenolate mofetil. From the 200 pregnancies exposed to tacrolimus no substantial teratogenic risk can be concluded. However, the use of these drugs does not require termination of pregnancy. A detailed fetal ultrasound examination should be offered to confirm normal morphologic development in cases of first-trimester exposure. Whether the immunosuppressant therapy of a transplanted woman who is stable on her medication should be changed is a question of risk–benefit assessment, because there is little experience in human pregnancies. 2.12.4 Selective immunosuppressants Schizophrenia and Mental Health Register Now 1. Chronic eczema is characterized by true polymorphism of rash elements, represented as microvesicles, microerosions, microcrusts with severe weeping as "serous wells", accompanied by itching. Neurodermatitis is characterized by itching, which passes ahead of the appearance of papular rashes. Eczema is also differed by the localization of lesions in limited areas of cutaneous covering. Dermographism is red in the presence of eczema, and in the presence of neurodermatitis it is white. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety. Access for professionals Atopic dermatitis (AD) is associated with colonization and increased risk of infections with Staphylococcus aureus. However, it's unclear how the skin microbiome during infancy affects the incidence of AD. This prospective study evaluated the effects of skin microbiome on the risk of developing AD during the first year of life. The study included 50 randomly selected infants from an Irish birth cohort study. Skin swabs from four sites relevant to AD were collected at 2 days and 2 and 6 months of age, with bacterial 16S rRNA gene sequencing and analysis performed directly from clinical samples. Patterns and changes in bacterial skin colonization were analyzed for association with the incidence of AD at 1 year. The types and diversity of bacteria in the skin microbiome changed significantly between sampling periods. In contrast to patients with established AD, occurrence of infantile AD was not associated with skin dysbiosis or colonization with S. aureus. Development of AD during the first year of life was associated with significant differences in bacterial communities detected in swabs from the antecubital fossa at 2 months. In particular, commensal staphylococci were significantly less abundant in infants who went on to develop AD. The skin microbiome was unrelated to mode of delivery or feeding method. Skin colonization with S. aureus does not appear to occur before the development of AD in infants. Early colonization with commensal staphylococci may have a protective effect against infantile AD. The authors emphasize the need for further research to understand the pathophysiology and mechanisms by which the skin microbiome affects the development of skin immunity and AD.
