Try Tai Chi to Prevent Falls Diseases and Conditions A-Z Diet, Food & Fitness Although not psoriasiform, mention is made of a recently described entity that clinically resembles lichen simplex chronicus or lichen amyloidosus but which has no histological similarities at all – pretibial pruritic papular dermatitis. The authors of that paper proposed that it was a response to chronic rubbing, possibly with other contributing factors such as xerosis, contact with irritants, and emotional distress. The lesions were red to flesh colored, pruritic papules 3–8 mm in diameter. There was a cobblestone appearance in some later lesions. Lesions were unilateral in 33/44 cases and bilateral in 11/44. The histology showed mild compact orthokeratosis, flattening of the rete ridges, superficial dermal fibrosis, and a mild to moderate superficial and mid-dermal infiltrate of lymphocytes, histiocytes and a few eosinophils. Stellate cells were present, probably a reflection of scratching. The published photomicrographs show some resemblance to pigmented purpuric dermatosis but there was no hemosiderin. Dermatology Index of Disease Severity(DIDS) is an efficient instrument for staging the severity of illness in inflammatory cutaneous diseases. The severity of illness for each patient was rated as 1 of 5 stages: 0, no evidence of clinical disease; I, limited disease; II, mild disease; III, moderate disease; and IV, severe disease. DIDS was applied as the measuring tool to determine the disease severity. Email Article ©1999-2018 petMD, LLC. All Rights Reserved Privacy Notice 3. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. Selected specialties Diseases of the skin and subcutaneous tissue (dermatology) Popular Health Centers Comments from Stan Fineman, MD: Skin barrier impairment in newborns is a potential risk factor for AD. Water can be a skin irritant and hard water, with high calcium carbonate, could cause even more irritation. This report from the Danish National Birth Cohort is unique since the geology of Denmark is unusual, with the country separated by a “main stationary line” caused by glaciers. The risk for developing AD increased by 5% for each 5 degrees of domestic water hardness, suggesting that using softer water could potentially be a primary prevention measure. Although the prevalence of AD increased in infants born in the colder seasons as expected, there was no synergism with hard water exposure. Maybe we should pay closer attention to the hardness of our local water. Epidemiology of atopic dermatitis. Carlsten C, Dimich-Ward H, Ferguson A, Watson W, Rousseau R, Dybuncio A, et al. QoL of most of (32.9%) our patients were moderate affected. The mean DLQI score in our study was 9.34, which was lower than previous report. Ermertcan AT found that DLQI score for patients in Turkey was 11.951. It can be explained by the limitation of patient selection. Patients included in the previous study were female only. In studies evaluating patients with chronic conditions, women consistently report poorer QoL than men7. Women were reported more pain8, more physical and psychological impairments9. Moreover, cases enrolled in the previous study were 43. When QoL affected by a specific entity was discussed, enough sample size will be more representative. Neurodermatitis must be differentiated from chronic eczema, lichen ruber planus, prurigo nodularis. Chronic eczema is characterized by true polymorphism of rash elements, represented as microvesicles, microerosions, microcrusts with severe weeping as "serous wells", accompanied by itching. Neurodermatitis is characterized by itching, which passes ahead of the appearance of papular rashes. Eczema is also differed by the localization of lesions in limited areas of cutaneous covering. Dermographism is red in the presence of eczema, and in the presence of neurodermatitis it is white. eczema treatment | eczema skin disease eczema treatment | home remedies for dermatitis eczema treatment | irritant dermatitis
