Policy Endoscopy (endoscopy) About NEJM Breaking the itch-scratch cycle of neurodermatitis is challenging. Treatment success depends on resisting the urge to rub or scratch the affected areas. Over-the-counter and prescription creams can help ease the itching. You'll also need to identify and eliminate factors that may be aggravating the problem. Unlike eczema, psoriasis, and other itchy skin conditions, people tend to develop just 1 or 2 patches that itch. While rare, neurodermatitis can cause several itchy patches. Lichen simplex chronicus (LSC) (also known as neurodermatitis[1]) is a skin disorder characterized by chronic itching and scratching.[2] The constant scratching causes thick, leathery, darkened, (lichenified) skin. This condition is associated with many factors, including the scratch-itch cycle, psychological stressors, and atopy. LSC is more common between ages 35 and 50 and is seen approximately twice as often in women compared to men.[3] Neurodermatitis is characterized by decreased serum levels of prostaglandin precursors, decrease of cAMP levels in leukocytes due to the defect of beta-adrenergic receptors, and also as a result of an increased activity of phosphodiesterase. It is believed that the consequence of cAMP decreased level can be an increased release of inflammatory mediators from leukocytes, including histamine, which causes decrease in the functional activity of T-lymphocytes via H2-receptor. This may explain IgE hyperproduction. The association with certain antibodies of tissue histocompatibility (HLA-A1, A9, B12, D24, DR1, DR7 and others) is found. According to the data of P. M. Alyeva (1993) DR5 antigen is a risk factor of disease development, and DR4 and DRw6 antigens are resistance factors. Most authors consider Vidal’s disease and disseminated neurodermatitis as an independent disease, but in patients with Vidal’s disease, the detection of immune phenomena, which are peculiar to atopic dermatitis, difference absence in the distribution of histocompatibility antigens in patients with the various prevalence of the process, the similarity in the disturbance of the metabolism of biogenic amines allow to consider disseminated neurodermatitis and Vidal’s disease as manifestations of one disease. The triggers for an episode of neurodermatitis differ from one person to the next and can change during the course of a lifetime. Several factors frequently play a part, which is why it is often difficult to identify the trigger or triggers. Food allergy YOUR EMAIL HAS BEEN SENT Patients with atopic dermatitis need safe and effective long-term treatments. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness.
