Pruritus precedes the scratching and may be precipitated by frustration, depression, and stress. The scratching then causes the lichenification and further itching, resulting in an “itch–scratch–itch” cycle that perpetuates the process. James G. Marks MD, Jeffrey J. Children were then followed up for the clinical development of AD. Interestingly the presence of S. aureus did not precede the development of AD—in fact, several commensal Staph species present at 2 months were associated with a reduced incidence of AD. This suggests that the relationship between the microbiome and skin inflammation is complicated, and that some commensal bacteria may even have a protective effect against eczema in infants. Previous reports have suggested higher rates of AD in areas with so-called hard water and among children born in fall or winter. This study looked for a possible synergistic relationship between these two AD risk factors. The researchers analyzed data on nearly 53,000 children from the Danish National Birth Cohort. National data sources were used to gather birth data and information on domestic water hardness, which differs across Danish regions. Physician diagnosis of AD and other characteristics were assessed in parental interviews when the children were 6 and 18 months old. Overall prevalence of AD was 15.0%. Prevalence ranged from 13.5% to 17.1% in regions with the softest versus hardest domestic water; relative prevalence of AD increased by 5% per 5-degree increase in water hardness. Hard water accounted for an estimated 2% of the population risk of AD. Relative prevalence of AD was 24% higher for children born in fall and 18% higher for those born in winter. There was no evidence of interaction between season of birth and water hardness. Both hard domestic water and fall/winter birth are associated with a higher prevalence of AD in early childhood. Risk of AD increases along with degree of water hardness, but there is no synergistic interaction between these two risk factors. Further studies are needed to see if water softening can reduce AD risk in infants. baby eczema | nummular dermatitis baby eczema | seborrheic dermatitis treatment baby eczema | contact dermatitis treatment
