Neurodermatitis is complicated by recurrent bacterial, viral and fungal infections, especially in those patients, who use hormonal ointments for a long time. Bacterial complications are represented by folliculitis, furunculosis, impetigo, and hydradenitis. The causative agent of these complications is usually Staphylococcus aureus, less epidermal one, white staphylococci or streptococci, the source of which are centers of chronic infection. The development of complications is accompanied by chill, increase of body temperature, sweating, increased congestion and itching. Peripheral lymph nodes are enlarged, painless. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. Board Prep Plus Psychic emotional stresses contribute to an exacerbation of neurodermatitis in almost one third of patients. Among other factors endocrine changes (pregnancy, menstrual cycle disturbance), drugs (antibiotics), prophylactic immunizations and others are noted. Foci of chronic infection in ENT-organs, digestive and urogenital systems, as well as bacterial colonization of skin have the great importance. An activation of these foci often leads to the exacerbation of the underlying disease. The severity of illness for each patient was rated as 1 of 5 stages: 0, no evidence of clinical disease; I, limited disease; II, mild disease; III, moderate disease; and IV, severe disease. DIDS was applied as the measuring tool to determine the disease severity. The itch can be intermittent, but also so intense that the affected person scratches or rubs the itchy patch frequently. The intensity is especially when the person is relaxing or sleeping, which can cause sleep deprivation. Frequently Asked Questions The impact of neurodermatitis on sexual function is of interest because the disease can limit a person's ability to perform essential life tasks and social roles. Moreover, sexual dysfunction may also cause significant personal distress and negatively impact quality of life (Thaipisuttikul, 1998; Abeck, 2005; Chuh et al, 2006; Burgin, 2008). In our study, we used the DLQI to evaluate the quality of life of patients. The mean DLQI score of 11.95 is similar to the mean scores of 11.20 and 10.53 reported for atopic dermatitis and psoriasis, respectively (Basra et al, 2008). The scores on question 9 of the DLQI showed that neurodermatitis affected sexual function in our patients. Consistent with this observation, previous studies have revealed a higher prevalence of sexual dysfunction in patients with chronic pain (Monga et al, 1998; Ambler et al, 2001). Sexual dysfunction in female patients with fibromyalgia has been demonstrated (Tikiz et al, 2005), and sexual status and erectile dysfunction in patients with chronic obstructive pulmonary disease have been suggested (Koseoglu et al, 2005). Sexual dysfunction studies on other systemic diseases, such as diabetes mellitus, chronic liver disease, allergic rhinoconjunctivitis, epilepsy, chronic renal failure, and multiple sclerosis have also been performed (Ermertcan, 2009).
