Kinetic studies in lichen simplex chronicus have shown epidermal cell proliferation similar to that seen in psoriasis although the transit time of the cells is not as fast. There is also an increase in mitochondrial enzymes in keratinocytes and in the number of melanocytes in the basal layer. Although these kinetic aspects are known, there is still no explanation for the pathogenesis of these plaques and the underlying pruritus; it is apparent, however, that self-induced trauma plays an important localizing role. Assessment Get Help for Migraine Relief Patients with atopic dermatitis need safe and effective long-term treatments. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. Genetic predisposition, environmental factors as well as inflammation taking place in the skin play a role. This reduced skin barrier is caused by a deficiency, an imbalance or defective functioning of substances (e.g. proteins or fats) that are responsible for the formation of the upper layer of the skin. As a result, more water evaporates and the skin becomes dry. In addition, the skin becomes more permeable to allergens and other environmental substances. These can trigger inflammatory reactions.  The distinctive features of the third age period (phase of puberty and adulthood) are: Q10 .713 Minshall EM Immunity Avoiding specific foods as appropriate if there is concomitant food allergy Allergic Skin Disorders In other projects Print the entire contents of article Get Involved in Research For Medical Professionals Sign in to NCBI NEJM Group Amazon Try Prime Please confirm that you would like to log out of Medscape.
