In some cases, neurodermatitis is associated with other skin conditions — such as dry skin, eczema or psoriasis. Stress and anxiety can trigger itching too. Interleukin (IL)-31 is involved in the pathogenesis of AD, specifically including the symptom of pruritus. The humanized monoclonal antibody nemolizumab inhibits IL-31 signaling via binding to interleukin-31 receptor A. This phase 2 randomized trial evaluated the safety and efficacy of nemolizumab in patients with AD. The 12-week study included 264 patients with moderateto- severe AD that did not respond to topical agents. Patients were assigned to receive subcutaneous nemolizumab 0.1, 0.5, or 2.0 mg/kg or placebo every 4 weeks. (An exploratory group received nemolizumab 2.0 mg/kg every 8 weeks.) There were 216 study completers. Percentage change on a pruritus visual analog scale in the 4-week treatment groups was -43.7% with the 0.1 mg/kg dose of nemolizumab, -59.8% with the 0.5 mg/kg dose, and -63.1% with the 2.0 mg/kg dose, compared to -20.9% with placebo. Changes on the Eczema Area and Severity Index were -23.0%, -42.3%, -40.9% in the three nemolizumab dose groups compared to -26.6% with placebo. Changes in body surface area affected were -7.5%, -20.0%, -19.4%, and -15.7%, respectively. Treatment discontinuation rate was 13% in the nemolizumab 20 mg/kg dose group and 17% in all other groups. At all monthly doses studied, nemolizumab reduced pruritus scores in patients with moderate to severe AD. The study supports an approach targeting IL-31 receptor A in patients with AD. Within its limitations, the study suggests that a nemolizumab dose of 0.5 mg/kg every 4 weeks provides the best risk-benefit profile. Programs What's the most likely cause of my itching? Approximately two-thirds of the patients were followed for at least 2 years and the rest were followed for at least 5 years. From ages 2 to 26 years, more than 80% of patients reported having continued symptoms and/or use of topical medications to control symptoms. By age 20, approximately half of the patients had experienced at least one 6-month symptom- and medication-free period. Living in southern states, having a relative with an atopic illness, and exposure to pollen, wool, pets, cigarettes, fumes, some foods or drinks, and soaps/detergents were linked to persistent symptoms. They are more suitable for use when the patient has Liver-Yin and Blood deficiency, and disturbance of the Liver-Wind, such as seen in pruritus in elderly people, skin diseases starting or worsening in stressful situations, itchy rashes in the evening, in the night or after menstruation, and chronic skin diseases. Since external Wind and internal Wind often coexist—for instance, the itch may not only be worse in the above conditions, but also when the weather changes, after taking spicy food or during cold infections—therefore substances that expel external Wind and those that extinguish internal Wind can be used together. Ending Sexual Harassment in Academic Medicine  V.J. Dzau et al. Demodectic Mange in Dogs Neurodermatitis is characterized by decreased serum levels of prostaglandin precursors, decrease of cAMP levels in leukocytes due to the defect of beta-adrenergic receptors, and also as a result of an increased activity of phosphodiesterase. It is believed that the consequence of cAMP decreased level can be an increased release of inflammatory mediators from leukocytes, including histamine, which causes decrease in the functional activity of T-lymphocytes via H2-receptor. This may explain IgE hyperproduction. The association with certain antibodies of tissue histocompatibility (HLA-A1, A9, B12, D24, DR1, DR7 and others) is found. According to the data of P. M. Alyeva (1993) DR5 antigen is a risk factor of disease development, and DR4 and DRw6 antigens are resistance factors. Most authors consider Vidal’s disease and disseminated neurodermatitis as an independent disease, but in patients with Vidal’s disease, the detection of immune phenomena, which are peculiar to atopic dermatitis, difference absence in the distribution of histocompatibility antigens in patients with the various prevalence of the process, the similarity in the disturbance of the metabolism of biogenic amines allow to consider disseminated neurodermatitis and Vidal’s disease as manifestations of one disease. Comments from Stan Fineman, MD: Skin barrier impairment in newborns is a potential risk factor for AD. Water can be a skin irritant and hard water, with high calcium carbonate, could cause even more irritation. This report from the Danish National Birth Cohort is unique since the geology of Denmark is unusual, with the country separated by a “main stationary line” caused by glaciers. The risk for developing AD increased by 5% for each 5 degrees of domestic water hardness, suggesting that using softer water could potentially be a primary prevention measure. Although the prevalence of AD increased in infants born in the colder seasons as expected, there was no synergism with hard water exposure. Maybe we should pay closer attention to the hardness of our local water. ABOUT About H&S Sustainability FAQ Seborrheic Dermatitis Hier finden Sie den kompletten Artikel als PDF-Dokument zum Download.
