Clinical By the end of the second period, an "atopic face" develops hyperpigmentation and wrinkles in the area of eyelids, imparting the child “tired appearance”. The rest of skin is also changed, but without clinically pronounced inflammation (dryness, dullness, defurfuration, dyschromia, infiltration). Neurodermatitis is characterized by seasonality disease course and is in the development of exacerbations in autumn and winter and a significant improvement or resolution of the process in summer, especially in the conditions of south. Donating The following features should be considered in the diagnosis of AD in accordance with the American Academy of Dermatology (AAD) 2014 Guidelines [3] : Treatment is aimed at reducing itching and minimizing existing lesions because rubbing and scratching exacerbate LSC. The itching and inflammation may be treated with a lotions or steroid cream (such as triamcinolone or Betamethasone) applied to the affected area of the skin.[7] Nighttime scratching can be reduced with sedatives and antihistamines.[2] SSRIs can effectively reduce the scratching associated with obsessive psychosomatic behaviors. However, changes in the regulation of the HPA axis can be related to several stress-related disorders. For example, chronic stress, major depression, schizophrenia, or anorexia nervosa seem to be associated with a hyperactive HPA axis, whereas a hyporeactive HPA axis is found more often in CFS, autoimmune processes, or PTSD. Since there are only few longitudinal data available to date on the relationship between HPA activity and stress-related disorders, the question of cause and effect remains to be answered in future research. One patient with neurodermatitis answered 3 questions and 1 patient with psoriasis vulgaris answered 2 questions. One patient with psoriasis did not list the age and 2 did not specify the gender. Table Table11 shows the demographic characteristics of both groups. Mean ages were 37.97 ± 14.45 (range 18-83, median 35) and 34.33 ± 13.84 (range 16-80, median 32) years for patients with neurodermatitis and psoriasis, respectively. The disease duration ranged from 0.03 to 480 months (mean 32.27, median 12) for patients with neurodermatitis, while the counterpart ranged from 0.5 to 552 months (mean 75.64, median 36). The two groups were matched for gender and age. The disease groups differed significantly in employment status, educational level, address, duration and age. Patients with neurodermatitis experienced high level of education and short disease duration. Comments by Chitra Dinakar, MD: This is certainly exciting: a therapeutic option that helps attenuate the signs and symptoms of AD, including pruritus and sleep effects; causes clinically meaningful reductions in patient-reported anxiety and depression; improves health-related quality of life; and can be administered subcutaneously weekly or every 2 weeks. In two independent 4-month randomized trials of identical design (to provide replication of results), injection-site reactions and conjunctivitis were the most frequent adverse effects. The etiology of the conjunctivitis was not clear; this finding was not seen in studies of dupilumab in asthma and sinusitis and polyposis. While trials of longer duration are needed to assess efficacy and other safety issues, a once every 2 weeks subcutaneous shot may be a tolerable option to these long-suffering patients. ABSTRACT: Neurodermatitis is a chronic disease affecting the patient's psychosocial status and quality of life. It is associated with a variety of psychologic problems, including demoralization, depression, anxiety, obsessive‐compulsive disorder, and sleep disturbances. Coexistence of sexual dysfunction, especially in women, with several systemic diseases has gained interest in recent years. In this study, we evaluated sexual function in female patients with neurodermatitis. We enrolled 89 women (43 patients, 46 controls) in the study. Quality of life was assessed with the Dermatology Life Quality Index (DLQI), and the Female Sexual Function Index (FSFI) was used to determine sexual function. Individuals with psychiatric disorders and/or those using antidepressants were excluded. The total DLQI score was 11.95 ± 5.65 in patients with neurodermatitis. The total FSFI score was significantly lower in patients compared with healthy controls (22.76 ± 5.31 and 28.83 ± 3.50, respectively; P = .001). Domain scores of FSFI (desire, arousal, lubrication, orgasm, and satisfaction) except pain were significantly lower in patients with neurodermatitis (P = .001). The pain score was also lower in patients than controls, but the difference was not statistically significant (P = .073). Neurodermatitis may be associated with sexual dysfunction, and patients with neurodermatitis should be evaluated with regard to sexual function to provide a better quality of life. William D James, MD is a member of the following medical societies: American Academy of Dermatology, Society for Investigative Dermatology If you suspect any type of skin problem or skin disorder, consult your doctor, skin specialist or dermatologist immediately. Neurodermatitis can present itself with other common skin conditionsm like eczema and psoriasis. The prevalence rate for atopic dermatitis (AD) is 10-12% in children and 0.9% in adults. More recent information examining physician visits for AD in the United States from 1997-2004 estimates a large increase in office visits for AD occurred. In addition, blacks and Asians visit more frequently for AD than whites. Note that this increase involves all disease under the umbrella of AD and it has not been possible to allocate which type has increased so rapidly.
