Common triggers for neurodermatitis include stress, anxiety, and anything that irritates your skin. If this happens, you will need to treat it again to get clearing. See Workup for more detail. [20] One mechanism by which filaggrin defects may influence inflammation is by the release of epithelial cell‒derived cytokines, including TSLP, IL-25, and IL-33, which are all known to be up-regulated in the context of AD. [21, 22, 23, 24] TSLP has been shown to be a potent promoter of basophil and ILC2 responses in the skin, while IL-25 and IL-33 preferentially elicit ILC2s. [12, 13, 16] Although filaggrin is strongly linked to AD, mutations are only found in 30% of European patients, begging the question of whether other genetic variants may also be responsible for some of the findings in the pathogenesis of AD. Indeed, genetic variants of TSLP have been shown to interact with mutations in filaggrin to influence AD disease persistence in patients. Factors affecting skin barrier function might influence the risk of atopic dermatitis (AD). Previous reports have suggested higher rates of AD in areas with so-called hard water and among children born in fall or winter. This study looked for a possible synergistic relationship between these two AD risk factors. The researchers analyzed data on nearly 53,000 children from the Danish National Birth Cohort. National data sources were used to gather birth data and information on domestic water hardness, which differs across Danish regions. Physician diagnosis of AD and other characteristics were assessed in parental interviews when the children were 6 and 18 months old. Overall prevalence of AD was 15.0%. Prevalence ranged from 13.5% to 17.1% in regions with the softest versus hardest domestic water; relative prevalence of AD increased by 5% per 5-degree increase in water hardness. Hard water accounted for an estimated 2% of the population risk of AD. Relative prevalence of AD was 24% higher for children born in fall and 18% higher for those born in winter. There was no evidence of interaction between season of birth and water hardness. Both hard domestic water and fall/winter birth are associated with a higher prevalence of AD in early childhood. Risk of AD increases along with degree of water hardness, but there is no synergistic interaction between these two risk factors. Further studies are needed to see if water softening can reduce AD risk in infants. Tropical Dermatology in Tanzania Kim BS, Siracusa MC, Saenz SA, Noti M, Monticelli LA, Sonnenberg GF, et al. A randomized controlled trial by Armstrong et al demonstrated improved patient education and clinical outcome in patients who watched a video on atopic dermatitis (AD) compared with those who received a pamphlet. [33] The study emphasized the importance of lifestyle changes and daily care in the successful treatment of AD. The eczema appears on typical areas of skin, depending on the sufferer’s age. During infancy it mainly affects the face, the upper body, hands and extensor surfaces of the arms and legs. In small children, the episodes mainly occur in the hollows of the knees, elbows, on the face, neck and throat. Hand and foot eczema may additionally be observed in older children, adolescents and adults. The Effects of Multiple Sclerosis Van Leent EJ Request Username Atopic dermatitis (AD) is a chronic, pruritic inflammatory skin disease (see image below) of unknown origin that usually starts in early infancy, but also affects a substantial number of adults. AD is commonly associated with elevated levels of immunoglobulin E (IgE). That it is the first disease to present in a series of allergic diseases—including food allergy, asthma, and allergic rhinitis, in order—has given rise to the “atopic march” theory, which suggests that AD is part of a progression that may lead to subsequent allergic disease at other epithelial barrier surfaces. Symptoms can persist or begin in adulthood. It is also the most common cause of occupational skin disease in adults. This disease results from an interaction between susceptibility genes, the host's environment, pharmacological abnormalities, skin barrier defects, and immunological factors. New management approaches have evolved from advances in our understanding of the pathobiology of this common skin disorder.
