Exfoliative dermatitis Diseases of the nervous system (neurology) My Profile Our Spring Allergy Capitals report is an independent research project of AAFA. SOA Product Directory Atopic dermatitis (AD) is associated with colonization and increased risk of infections with Staphylococcus aureus. However, it's unclear how the skin microbiome during infancy affects the incidence of AD. This prospective study evaluated the effects of skin microbiome on the risk of developing AD during the first year of life. The study included 50 randomly selected infants from an Irish birth cohort study. Skin swabs from four sites relevant to AD were collected at 2 days and 2 and 6 months of age, with bacterial 16S rRNA gene sequencing and analysis performed directly from clinical samples. Patterns and changes in bacterial skin colonization were analyzed for association with the incidence of AD at 1 year. The types and diversity of bacteria in the skin microbiome changed significantly between sampling periods. In contrast to patients with established AD, occurrence of infantile AD was not associated with skin dysbiosis or colonization with S. aureus. Development of AD during the first year of life was associated with significant differences in bacterial communities detected in swabs from the antecubital fossa at 2 months. In particular, commensal staphylococci were significantly less abundant in infants who went on to develop AD. The skin microbiome was unrelated to mode of delivery or feeding method. Skin colonization with S. aureus does not appear to occur before the development of AD in infants. Early colonization with commensal staphylococci may have a protective effect against infantile AD. The authors emphasize the need for further research to understand the pathophysiology and mechanisms by which the skin microbiome affects the development of skin immunity and AD. The impact of neurodermatits and psoriasisi on sexual function had been studied previously. Inconsistent with our data, Mercan S found that neurodermatitis patients had more sexual problems than the psoriasis counterpart12. The difference may be due to the sample size. There were only 31 patients with neurodermatitis and 24 patients with psoriasis, while the counterpart in our study are 149 and 246 respectively. Another comment is related to the questionnaire applied in the survey. DLQI was applied in our study while Arizona Sexual Experience Scale in Mercan's study. Although DLQI has question(Q9) about sexual life, it do not specifically evaluate sexual problems. Gender-specific sexual function scales and proper sample size will be needed in future study. The Dermatology Life Quality Index (DLQI) has been used in 33 different skin conditions and is available in 55 languages. DLQI comprises 10 items, giving a sum score ranging between 0 and 306. Ten questions concerning symptoms, embarrassment, shopping/daily activities, clothes, social/leisure, sport, work or study, relationships, sexual difficulties and treatment. High DLQI scores imply low quality of life. The DLQI has been used in cross-sectional studies of different disorders, such as acne, atopic dermatitis and psoriasis. Ten items were explained to all subjects and data collectors helped them to complete the survey questions. The authors of that paper proposed that it was a response to chronic rubbing, possibly with other contributing factors such as xerosis, contact with irritants, and emotional distress. The lesions were red to flesh colored, pruritic papules 3–8 mm in diameter. There was a cobblestone appearance in some later lesions. Lesions were unilateral in 33/44 cases and bilateral in 11/44. The histology showed mild compact orthokeratosis, flattening of the rete ridges, superficial dermal fibrosis, and a mild to moderate superficial and mid-dermal infiltrate of lymphocytes, histiocytes and a few eosinophils. Stellate cells were present, probably a reflection of scratching. The published photomicrographs show some resemblance to pigmented purpuric dermatosis but there was no hemosiderin.
