In cases involving long-term prenatal therapy in the second and/or third trimesters, the newborn should be monitored for transient renal insufficiency and hyperkalemia. This applies to tacrolimus, everolimus, and sirolimus. There are insufficient data concerning the safety of first-trimester use of sirolimus, everolimus, and mycophenolate mofetil. From the 200 pregnancies exposed to tacrolimus no substantial teratogenic risk can be concluded. However, the use of these drugs does not require termination of pregnancy. A detailed fetal ultrasound examination should be offered to confirm normal morphologic development in cases of first-trimester exposure. Whether the immunosuppressant therapy of a transplanted woman who is stable on her medication should be changed is a question of risk–benefit assessment, because there is little experience in human pregnancies. Scratching will temporarily get rid of the itching but it damages the skin, which in turn encourages itching. The aim of treatment is to break this vicious circle between itching, scratching and deterioration in the skin's appearance. QoL of most of (32.9%) our patients were moderate affected. The mean DLQI score in our study was 9.34, which was lower than previous report. Ermertcan AT found that DLQI score for patients in Turkey was 11.951. It can be explained by the limitation of patient selection. Patients included in the previous study were female only. In studies evaluating patients with chronic conditions, women consistently report poorer QoL than men7. Women were reported more pain8, more physical and psychological impairments9. Moreover, cases enrolled in the previous study were 43. When QoL affected by a specific entity was discussed, enough sample size will be more representative. Resident International Grant Viral Skin Diseases Table 6 Factors affecting skin barrier function might influence the risk of atopic dermatitis (AD). Previous reports have suggested higher rates of AD in areas with so-called hard water and among children born in fall or winter. This study looked for a possible synergistic relationship between these two AD risk factors. The researchers analyzed data on nearly 53,000 children from the Danish National Birth Cohort. National data sources were used to gather birth data and information on domestic water hardness, which differs across Danish regions. Physician diagnosis of AD and other characteristics were assessed in parental interviews when the children were 6 and 18 months old. Overall prevalence of AD was 15.0%. Prevalence ranged from 13.5% to 17.1% in regions with the softest versus hardest domestic water; relative prevalence of AD increased by 5% per 5-degree increase in water hardness. Hard water accounted for an estimated 2% of the population risk of AD. Relative prevalence of AD was 24% higher for children born in fall and 18% higher for those born in winter. There was no evidence of interaction between season of birth and water hardness. Both hard domestic water and fall/winter birth are associated with a higher prevalence of AD in early childhood. Risk of AD increases along with degree of water hardness, but there is no synergistic interaction between these two risk factors. Further studies are needed to see if water softening can reduce AD risk in infants. Study notes Where Can I Get More Information About the Spring 2018 Rankings?
