Arch Dermatol. 1999; 135: 583-586 Pharmacology and toxicology The itch can be intermittent, but also so intense that the affected person scratches or rubs the itchy patch frequently. The intensity is especially when the person is relaxing or sleeping, which can cause sleep deprivation.
World Congress of Dermatology Transient recurrent immunodeficiency is considered to be one of the factors, predisposing the development of atopy. In the skin of patients decrease in number of Langerhans cells and reduction of HLA-DR antigen expression on them, increase of the number of Langerhans cells with IgE-receptors are detected. From the immune disorders elevated levels of serum IgE are observed that is believed to be genetically determined, although this sign is not observed in all patients with neurodermatitis; the lack of T-lymphocytes, especially having suppressor properties, is caused probably by defective beta-adrenergic receptors. The number of B-cells is normal, but there is a slight increase in the number of B-lymphocytes, bearing receptors for Fc-fragment of IgE. Neutrophilic chemotaxis, a function of natural killer cells, production of interleukin-1 by monocytes of patients in comparison with control observations are reduced. The presence of immune system defect serves apparently as one of the major causes of patients’ susceptibility to infectious diseases. Pathogenetic importance of non-bacterial allergy on allergens of an infectious origin is shown. Neurovegetative disturbances are given with both etiological and aggravating significances in the course of the disease.
Related Conditions and Diseases MyDermPath+ Become an Affiliate Neurodermatitis is complicated by recurrent bacterial, viral and fungal infections, especially in those patients, who use hormonal ointments for a long time. Bacterial complications are represented by folliculitis, furunculosis, impetigo, and hydradenitis. The causative agent of these complications is usually Staphylococcus aureus, less epidermal one, white staphylococci or streptococci, the source of which are centers of chronic infection. The development of complications is accompanied by chill, increase of body temperature, sweating, increased congestion and itching. Peripheral lymph nodes are enlarged, painless.

IgE receptors. Support Groups The term ‘prurigo nodularis’ (see p. 670) is used for lesions with a nodular clinical appearance and prominent epidermal hyperplasia of pseudoepitheliomatous rather than psoriasiform type. At times, lesions with overlapping clinical and histopathological features of both lichen simplex chronicus and prurigo nodularis are found. Prurigo nodularis is one condition in which the histological picture often lags behind the clinical appearance. The author has seen numerous cases, regarded by experienced dermatologists as prurigo nodularis, in which the histological picture was that of lichen simplex chronicus.

In the dermis perivascular infiltrates of lymphocytes with an admixture of neutrophils are present. In older foci, except acanthosis, hyperkeratosis and parakeratosis, sometimes, spongiosis are expressed. In the dermis dilatation of capillaries with endothelial swelling, around which small infiltrates of the lymphatic and histiocytic nature with a significant number of fibroblasts can be seen. In the central part of lesion focus pigment is not detected in the basal layer, whereas in peripheral parts of it, particularly in older foci of lichenification, melanin amount is increased.
