Detailed investigations with a dermatologist can provide the necessary information. Q4 .704 Comments by Chitra Dinakar, MD: This is certainly exciting: a therapeutic option that helps attenuate the signs and symptoms of AD, including pruritus and sleep effects; causes clinically meaningful reductions in patient-reported anxiety and depression; improves health-related quality of life; and can be administered subcutaneously weekly or every 2 weeks. In two independent 4-month randomized trials of identical design (to provide replication of results), injection-site reactions and conjunctivitis were the most frequent adverse effects. The etiology of the conjunctivitis was not clear; this finding was not seen in studies of dupilumab in asthma and sinusitis and polyposis. While trials of longer duration are needed to assess efficacy and other safety issues, a once every 2 weeks subcutaneous shot may be a tolerable option to these long-suffering patients. Air Pollution A thick layer of compact orthokeratosis (resembling that seen on normal palms and soles) is present, overlying hypergranulosis. Focal zones of parakeratosis are sometimes interspersed with the orthokeratosis, but there is not the confluent parakeratosis of psoriasis. The epidermis shows psoriasiform hyperplasia with thicker rete ridges of less even length than in psoriasis. Epidermal thickness and volume are greater than in psoriasis. Minimal papillomatosis is sometimes present in a few areas. Focal excoriation is another change that may be seen in lichen simplex chronicus. Perioral Dermatitis In conclusion, neurodermatits moderately affected the QoL of the patients. People should pay more attention to the chronic disease, although it is not life-threatening. There are two limitations in this study: we were unable to compare QoL of patients before and after treatment, which could be incorporated in the planning of future studies. Another comment is related to the questionnaire itself, part of patients worried about the impact of diet on disease which does not exist in DLQI because of the different culture background. Comments by Stan Fineman, MD: This birth cohort study analyzed data from skin samples at age 2 days, 2 months, and 6 months, with gene sequencing to determine microbiome changes. Children were then followed up for the clinical development of AD. Interestingly the presence of S. aureus did not precede the development of AD—in fact, several commensal Staph species present at 2 months were associated with a reduced incidence of AD. This suggests that the relationship between the microbiome and skin inflammation is complicated, and that some commensal bacteria may even have a protective effect against eczema in infants. The causes of neurodermatitis are many and varied. Genetic predisposition, environmental factors as well as inflammation taking place in the skin play a role. This reduced skin barrier is caused by a deficiency, an imbalance or defective functioning of substances (e.g. proteins or fats) that are responsible for the formation of the upper layer of the skin. As a result, more water evaporates and the skin becomes dry. In addition, the skin becomes more permeable to allergens and other environmental substances. These can trigger inflammatory reactions.  Contact allergy card Article Info Burns 2 star2 star (0%) Bernice R Krafchik, MBChB, FRCPC is a member of the following medical societies: American Academy of Dermatology, American Dermatological Association, Canadian Medical Association, College of Physicians and Surgeons of Ontario, Royal College of Physicians and Surgeons of Canada, and Society for Pediatric Dermatology IN ATOPIC DERMATITIS, INFLAMMATION IS AT THE CORE CDC:  “Syphilis-CDC Fact Sheet,” “Genital Herpes-CDC Fact Sheet.” Quality care, guidelines and reporting Chronic and relapsing course Start Here Featured Topics Amazon Currency Converter Enhanced expression of B7.2 (CD86) in patients with atopic dermatitis: a potential role in the modulation of IgE synthesis.
