Specialties That itching feeling that is normal for dry patches of skin may have no effect on some people and a chronic effect on others. The incidence of neurodermatitis, which is also known as atopic dermatitis or atopic eczema, and of psoriasis have increased in the last ten to fifteen years. Neurodermatitis is characterized by chronic itching or scaling of the skin. It starts with a patch of itchy skin, which leads to scratching. Scratching makes the affected area even itchier and the sufferer scratches a little more until he or she scratches out of habit. Chronic itching and scratching can cause the affected area to become thick and leathery. It is therefore essential to break this itch-scratch cycle. Sexual Conditions Atopic dermatitis (AD) (or atopic eczema) is a chronic skin condition that affects the quality of life of both adults and children. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety. The usual treatment is a topical corticosteroid but if thick plaques are present, potent corticosteroids under occlusion or triamcinolone injections may be needed. Clinical implications are discussed. Focal zones of parakeratosis are sometimes interspersed with the orthokeratosis, but there is not the confluent parakeratosis of psoriasis. The epidermis shows psoriasiform hyperplasia with thicker rete ridges of less even length than in psoriasis. Epidermal thickness and volume are greater than in psoriasis. Minimal papillomatosis is sometimes present in a few areas. Focal excoriation is another change that may be seen in lichen simplex chronicus.
