Neurodermatitis is complicated by recurrent bacterial, viral and fungal infections, especially in those patients, who use hormonal ointments for a long time. Bacterial complications are represented by folliculitis, furunculosis, impetigo, and hydradenitis. The causative agent of these complications is usually Staphylococcus aureus, less epidermal one, white staphylococci or streptococci, the source of which are centers of chronic infection. The development of complications is accompanied by chill, increase of body temperature, sweating, increased congestion and itching. Peripheral lymph nodes are enlarged, painless. Cool compress: Applying a cool compress or soaking the area for 5 minutes before you apply a corticosteroid can help the medicine penetrate thickened skin more easily — and reduce the itchiness. The primary immune dysfunction hypothesis invokes an imbalance in the T cell subsets, with Th2 cells predominating; this results in the production of type 2 cytokines such as interleukin (IL)–4, IL-5, and IL-13, causing an increase in IgE from plasma cells. Later, in persons with chronic AD, the Th1 cells have been shown to predominate. More recently, Th17 cells have been found to be elevated in patients with AD. [9] Although primarily considered a Th2 cell‒associated cytokine-mediated disease, the precise contributions of Th1 and Th17 cell responses remain to be fully defined. Tennessee MedicineNet Patient advocates Ochsendorf F Neurodermatitis is a skin condition that’s also known as lichen simplex chronicus. It causes patches of itchy skin that lead to scratching, which makes you feel even itchier. The condition isn’t contagious. It may be due to something that irritates your skin, like a bug bite or tight clothes that trigger the itch-scratch cycle. Advertise Your Products Corresponding Author What would you like to print? Factors affecting skin barrier function might influence the risk of atopic dermatitis (AD). Previous reports have suggested higher rates of AD in areas with so-called hard water and among children born in fall or winter. This study looked for a possible synergistic relationship between these two AD risk factors. The researchers analyzed data on nearly 53,000 children from the Danish National Birth Cohort. National data sources were used to gather birth data and information on domestic water hardness, which differs across Danish regions. Physician diagnosis of AD and other characteristics were assessed in parental interviews when the children were 6 and 18 months old. Overall prevalence of AD was 15.0%. Prevalence ranged from 13.5% to 17.1% in regions with the softest versus hardest domestic water; relative prevalence of AD increased by 5% per 5-degree increase in water hardness. Hard water accounted for an estimated 2% of the population risk of AD. Relative prevalence of AD was 24% higher for children born in fall and 18% higher for those born in winter. There was no evidence of interaction between season of birth and water hardness. Both hard domestic water and fall/winter birth are associated with a higher prevalence of AD in early childhood. Risk of AD increases along with degree of water hardness, but there is no synergistic interaction between these two risk factors. Further studies are needed to see if water softening can reduce AD risk in infants. Pharmacology and toxicology Clinical Cases Allergic march Ultrafast Delivery In terms of AD-associated itch, Th2 cells are known to be significant sources of the itch-inducing cytokine or pruritogen IL-31. [61] Emerging clinical trials data indicate that blocking this pathway may be a key mechanism by which atopic itch can be treated clinically. Additionally, a 2017 study identified that neuronal, rather than immune, signaling of the type 2 cytokines IL-4 and IL-13 critically regulate AD-associated itch. [62] Indeed, the dual IL-4 and IL-13 blocker, dupilumab, has emerged as a highly effective treatment for AD, which received FDA approval in March of 2017. Thus, blocking cytokine-nerve interactions with targeted biologic therapies has emerged as a novel therapeutic strategy in AD.
