Scratching will temporarily get rid of the itching but it damages the skin, which in turn encourages itching. The aim of treatment is to break this vicious circle between itching, scratching and deterioration in the skin's appearance. Various measures may be helpful in this situation, e.g. cooling, skin-friendly and breathable fabrics, compresses and dressings with black tea, table salt or ointments. When this is not the case, eczema is usually symmetrical and does not extend beyond the areola (see Fig. 12.6). The possibility of an artefactual syndrome (see Ch. 17) should not be overlooked. Neurodermatitis is a chronic, relapsing inflammatory disorder, characterized by rashes of nodular (papular) elements on skin that are prone to fusion and formation of foci of infiltration and lichenification, accompanied by intense itching. Neurodermatitis, also known as lichen simplex chronicus, is a common chronic skin disease, affecting up to 12% of the total population, and women are more affected than men1. The disease is characterized by lichenificated plaque as a result of excessive scratching. Neck, elbow, ankles, vulva, eyelid even faces are the most common affected sites. Although neurodermatitis is not life-threatening, it can produce an important psychosocial burden. It has been suggested that patients with neurodermatitis suffer from depression, anxiety and other treatable psychological disorders2. Negative emotional states are the main personality component of patients (greater tendency to pain avoidance, greater dependency on other peoples' desires, and more conforming and dutiful)3. Patients with neurodermatitis tend to have poor social skills or interpersonal resources and a lack of flexibility. Neurodermatitis may be associated with sleep disturbance and sexual dysfunction4. All these data constitute a growing body of evidence indicating a negative impact of neurodermatitis on patients' quality of life (QoL). SEND TO ANOTHER RECIPIENT DONE Home » Dog Conditions Once the skin is scratched, a vicious itch-scratch-itch cycle develops. The more the skin is scratched, rubbed, or even touched, the more it itches. The itch can become so intense that it disrupts sleep. Higher than average medicine use Should I seek medical care? Explore Research Labs Constipation Narrow neurodermatitis is more common in adult males and is characterized by one or more lesions in the form of plaques of various sizes and shapes on the skin of neck, reproductive organs (anogenital area), elbow and popliteal creases. Plaques are symmetrically arranged delimitating relatively clearly from unaffected skin by the area of hyperpigmentation. In focus areas skin is dry, infiltrated, with an exaggerated picture, more pronounced in the center. On the periphery of foci small (pinhead) polygonal flat papules with a glossy surface of a brownish-red or pink color are located. Tacrolimus is a macrolide obtained from Streptomyces, and is widely used as an immunosuppressant in transplant medicine. Skin diseases such as neurodermatitis are another indication; this will be discussed elsewhere (see Chapter 2.17). Certain advantages and disadvantages of tacrolimus in pregnancy are discussed compared to cyclosporine: rejection and hypertension are less common with tacrolimus, and the necessary dosage of prednisolone is lower. On the other hand, gestational diabetes occurs more often, as does transient hyperkalemia and transient reduction of kidney function in the newborn. Jain (1997) even reported on a newborn with anuria which lasted 36 hours. As with other immunosuppressant drugs, pre-eclampsia, prematurity, low birth weight, and cesarian births were seen at greater incidence. Sign up for the My MedlinePlus newsletter What's this? Protect Yourself from a Bone Fracture Dermatology Daily In our study, we evaluated sexual function using question 9 of the DLQI and the FSFI. According to question 9 of the DLQI, sexual function was affected in our patients with neurodermatitis. When we looked at the total and individual scores from the FSFI, the total score was significantly decreased in female patients with neurodermatitis compared with that in controls. Analyses of each domain showed that scores for desire, arousal, lubrication, orgasm, and satisfaction were significantly decreased in these patients. The pain score was also lower in the patient group than in the control group, although the difference was not statistically significant. As observed in Niemeier et al (1997) and Mercan et al (2008), we conclude that neurodermatitis often results in sexual dysfunction. We also conclude that neurodermatitis has effects particularly on the desire, arousal, lubrication, orgasm, and satisfaction components of sexual function in female patients. Changes like those of lichen simplex chronicus may be superimposed on other dermatoses such as lichen planus (hypertrophic lichen planus), mycosis fungoides, actinic reticuloid, and eczematous dermatitides including atopic dermatitis. These changes are particularly prominent in some solar keratoses of the hands or forearms. Would you like to tell us about a lower price? Itching typically precedes the rash of lichen simplex chronicus. If the area looks infected, your dermatologist will swab the area. Testing the sample on the swab can tell your dermatologist whether you have an infection. Contributor Information and Disclosures Screen Reader: Supported Quality of life of the patients with neurodermatitis was assessed with the Turkish version of the Dermatology Life Quality Index (DLQI). The DLQI is a 10‐item self‐administered instrument that measures physical function, work status, social and sport activities, depression, anxiety, pain, and well‐being. Question 9 of the DLQI is associated with sexual function (Öztürkcan et al, 2006). It has been used worldwide in more than 45,000 patients with at least 33 different diseases (Basra et al, 2008). In the DLQI, higher scores signify poorer quality of life, and patients with dermatologic diseases have scores typically ranging from 5 to 15.
