Regional variations occur in lichen simplex chronicus. Epidermal hyperplasia is usually quite mild in lesions on the lip, while vertical-streaked collagen is unusual in lesions on the scalp or in mucocutaneous regions such as the vulva and perianal area. Role of inhalant allergens in atopic dermatitis. Conditions caused by hypersensitivity of the immune system to something in the environment that usually causes little or no problem in most people. Symptoms may include red eyes, an itchy rash, runny nose, shortness of breath, or swelling. Factors affecting skin barrier function might influence the risk of atopic dermatitis (AD). Previous reports have suggested higher rates of AD in areas with so-called hard water and among children born in fall or winter. This study looked for a possible synergistic relationship between these two AD risk factors. The researchers analyzed data on nearly 53,000 children from the Danish National Birth Cohort. National data sources were used to gather birth data and information on domestic water hardness, which differs across Danish regions. Physician diagnosis of AD and other characteristics were assessed in parental interviews when the children were 6 and 18 months old. Overall prevalence of AD was 15.0%. Prevalence ranged from 13.5% to 17.1% in regions with the softest versus hardest domestic water; relative prevalence of AD increased by 5% per 5-degree increase in water hardness. Hard water accounted for an estimated 2% of the population risk of AD. Relative prevalence of AD was 24% higher for children born in fall and 18% higher for those born in winter. There was no evidence of interaction between season of birth and water hardness. Both hard domestic water and fall/winter birth are associated with a higher prevalence of AD in early childhood. Risk of AD increases along with degree of water hardness, but there is no synergistic interaction between these two risk factors. Further studies are needed to see if water softening can reduce AD risk in infants. The relationships between DLQI scores and clinical, social and demographic factors were analyzed using multiple logistic regression. Although patients living in the rural region were 5.88 times more likely to have a high score when compared with that living in the urban region, the difference was not significance. Scores were not associated with gender, education, duration, employment status and age. The authors of that paper proposed that it was a response to chronic rubbing, possibly with other contributing factors such as xerosis, contact with irritants, and emotional distress. The lesions were red to flesh colored, pruritic papules 3–8 mm in diameter. There was a cobblestone appearance in some later lesions. Lesions were unilateral in 33/44 cases and bilateral in 11/44. The histology showed mild compact orthokeratosis, flattening of the rete ridges, superficial dermal fibrosis, and a mild to moderate superficial and mid-dermal infiltrate of lymphocytes, histiocytes and a few eosinophils. Stellate cells were present, probably a reflection of scratching. The published photomicrographs show some resemblance to pigmented purpuric dermatosis but there was no hemosiderin.
