Lichen simplex chronicus (also called “neurodermatitis”) is a chronic eczematous eruption of the skin that is the result of scratching (Fig. 8.13). Pruritus precedes the scratching and may be precipitated by frustration, depression, and stress. The scratching then causes the lichenification and further itching, resulting in an “itch–scratch–itch” cycle that perpetuates the process.
Factors affecting skin barrier function might influence the risk of atopic dermatitis (AD). Previous reports have suggested higher rates of AD in areas with so-called hard water and among children born in fall or winter. This study looked for a possible synergistic relationship between these two AD risk factors. The researchers analyzed data on nearly 53,000 children from the Danish National Birth Cohort. National data sources were used to gather birth data and information on domestic water hardness, which differs across Danish regions. Physician diagnosis of AD and other characteristics were assessed in parental interviews when the children were 6 and 18 months old. Overall prevalence of AD was 15.0%. Prevalence ranged from 13.5% to 17.1% in regions with the softest versus hardest domestic water; relative prevalence of AD increased by 5% per 5-degree increase in water hardness. Hard water accounted for an estimated 2% of the population risk of AD. Relative prevalence of AD was 24% higher for children born in fall and 18% higher for those born in winter. There was no evidence of interaction between season of birth and water hardness. Both hard domestic water and fall/winter birth are associated with a higher prevalence of AD in early childhood. Risk of AD increases along with degree of water hardness, but there is no synergistic interaction between these two risk factors. Further studies are needed to see if water softening can reduce AD risk in infants.

Indistinct seasonality of exacerbations of the disease. Sexual dysfunction in many dermatological diseases has recently attracted significant attention, because of its negative impact on quality of life. The impact of neurodermatits and psoriasisi on sexual function had been studied previously. Inconsistent with our data, Mercan S found that neurodermatitis patients had more sexual problems than the psoriasis counterpart12. The difference may be due to the sample size. There were only 31 patients with neurodermatitis and 24 patients with psoriasis, while the counterpart in our study are 149 and 246 respectively. Another comment is related to the questionnaire applied in the survey. DLQI was applied in our study while Arizona Sexual Experience Scale in Mercan's study. Although DLQI has question(Q9) about sexual life, it do not specifically evaluate sexual problems. Gender-specific sexual function scales and proper sample size will be needed in future study.

Narrow neurodermatitis is more common in adult males and is characterized by one or more lesions in the form of plaques of various sizes and shapes on the skin of neck, reproductive organs (anogenital area), elbow and popliteal creases. Plaques are symmetrically arranged delimitating relatively clearly from unaffected skin by the area of hyperpigmentation. In focus areas skin is dry, infiltrated, with an exaggerated picture, more pronounced in the center. On the periphery of foci small (pinhead) polygonal flat papules with a glossy surface of a brownish-red or pink color are located.
