However, it's unclear how the skin microbiome during infancy affects the incidence of AD. This prospective study evaluated the effects of skin microbiome on the risk of developing AD during the first year of life. The study included 50 randomly selected infants from an Irish birth cohort study. Skin swabs from four sites relevant to AD were collected at 2 days and 2 and 6 months of age, with bacterial 16S rRNA gene sequencing and analysis performed directly from clinical samples. Patterns and changes in bacterial skin colonization were analyzed for association with the incidence of AD at 1 year. The types and diversity of bacteria in the skin microbiome changed significantly between sampling periods. In contrast to patients with established AD, occurrence of infantile AD was not associated with skin dysbiosis or colonization with S. aureus. Development of AD during the first year of life was associated with significant differences in bacterial communities detected in swabs from the antecubital fossa at 2 months. In particular, commensal staphylococci were significantly less abundant in infants who went on to develop AD. The skin microbiome was unrelated to mode of delivery or feeding method. Skin colonization with S. aureus does not appear to occur before the development of AD in infants. Early colonization with commensal staphylococci may have a protective effect against infantile AD. The authors emphasize the need for further research to understand the pathophysiology and mechanisms by which the skin microbiome affects the development of skin immunity and AD. 2008;358(14): 1483-1494. © 2016 Massachusetts Medical Society. All lesions in the patients were localized on their backs, where neurodermatitis lesions are commonly seen. We wanted to eliminate the localization factor from sexual function and evaluate the impacts of this skin disease alone on sexual function. Therefore, patients with anogenital localization were excluded from this study. Routine blood count, biochemistry, erythrocyte sedimentation rate, and urinalysis were performed for each participant. The functional integrity of the hypothalamo‐pituitary‐gonadal axis was examined in all participants by measuring the levels of follicle‐stimulating hormone, luteinizing hormone, estradiol, free testosterone, prolactin, and sex hormone–binding globulin. Participants suffering from pulmonary, hepatic, hematologic, renal, and endocrine diseases such as diabetes mellitus, as well as thyroid function disorders and inflammatory diseases such as rheumatoid arthritis, were excluded from the study. Other exclusion criteria included pregnancy and gynecologic and systemic disorders that might affect female sexual function such as hormonal, muscular, neurologic, or cardiovascular diseases and psychologic disorders. Participants on antidepressants and other drugs that are known to interfere with sexual function and postmenopausal women receiving oral or vaginal estrogen therapy for the prevention of climacteric symptoms or vaginal dryness were also excluded from the study. Other general skin conditions may cause concern when the first, or only, manifestation of the condition occurs on the nipple or areola. Figure 12.10 shows a patient with vitiligo confined to the areola that showed such concern. In healthy individuals, balance exists between important subsets of T cells (eg, Th1, Th2, Th17, Th22). The primary immune dysfunction hypothesis invokes an imbalance in the T cell subsets, with Th2 cells predominating; this results in the production of type 2 cytokines such as interleukin (IL)–4, IL-5, and IL-13, causing an increase in IgE from plasma cells. Later, in persons with chronic AD, the Th1 cells have been shown to predominate. More recently, Th17 cells have been found to be elevated in patients with AD. [9] Although primarily considered a Th2 cell‒associated cytokine-mediated disease, the precise contributions of Th1 and Th17 cell responses remain to be fully defined.
