Find out what it is and how to treat with our in-depth guide.Neurodermatitis is a condition that tends to affect more women than men, and is more prevalent among those aged 30-50. It is characterised by a persistent itch that can cause a host of related problems.Unfortunately, there’s very little that is known for sure about this skin condition. What causes neurodermatitis? The biggest question around neurodermatitis is its cause. And on that score scientists have no definite answers.Despite a noted prevalence among certain parts of the population, and a link to stress and anxiety, there is no definitive answer to what causes neurodermatitis. Here’s what we do know: neurodermatitis starts as a small patch of itchy skin. The itch can be very intense, and frequent, and it can become so uncomfortable that it disrupts sleep. The repetitive scratching or rubbing of the itchy skin means that the affected area eventually becomes thick and leathery, and may become infected if the skin is broken. While neurodermatitis isn’t contagious, the original site may develop into several itchy spots on the body.So how do you stop something when you don’t know what causes it?Treating neurodermatitisThe current approach to neurodermatitis treatment is targeted towards symptoms.Doctors try to reduce the effects of the condition by administering antihistamines, corticosteroids and other anti-inflammatories. The idea here is to reduce itching so that the skin can heal.In cases where anxiety or stress are suspected as contributing to the condition, anti-anxiety medication is also used. The key here is that neurodermatitis is best treated by a doctor. If you find yourself suffering from constant itching, your best bet is to visit your dermatologist. Bonnekoh B Other skin conditions. Wrapping the area in plastic, covering it with clothing or a corticosteroid tape, or wearing an Unna boot (gauze dressing that contains ingredients like zinc oxide to promote healing) can prevent you from scratching. This can be very helpful if you scratch while sleeping. Pronounced skin lines in the patches What are you doing to stop the itch? Patients with atopic dermatitis need safe and effective long-term treatments. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. gloves Handschuhe | Researchers & Academics PMC2370465 The top five most challenging places to live with spring allergies this year are: Philanthropy in Action Williams H, Flohr C. Though a lot more has been learned about immunopathogenesis of AD, recognition of itching and scratching as hallmarks of this disease remains true. Itching is often worse at night. Chronic sleep disturbance due to frequent scratching can have adverse effects on the patient and his/her immediate family members and is a source of significant stress. When subjects with AD get upset, they tend to itch even more, probably secondary to flushing of the skin due to vasodilatation induced by neurogenic peptides, followed by histamine and prostaglandin E2 (Ostlere, Cowen et al. 1995). Papules, lichenification, and eczematous lesions result from repeated skin trauma. Vesicles and erythema with excoriation of the skin are seen in acute eczema. Crusted lesions and pustules are seen in infected eczema. Allergens, irritants (wool, soap, detergents, heat and humidity with sweating), infections, and certain foods can worsen eczema, and such history helps establish the right diagnosis. Distribution of rash on the face and extensor surfaces in infants and young children, changing to mostly flexural involvement in older ages, is a classic finding of AD and one of the essential features required to make this diagnosis. Dry skin (xerosis), personal and family history of allergic diseases, palmar hyperlinearity, nipple eczema, kera-tosis pilaris, pityriasis alba, and non-specific hand or foot dermatitis are some of the other clinical manifestations of this condition (Leung and Bieber 2003; Leung, Jain et al. 2003). The impact of AD on self-esteem and social interactions of kids or adults with this condition cannot be underestimated (Lapidus and Kerr 2001).
