The triggers for an episode of neurodermatitis differ from one person to the next and can change during the course of a lifetime. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety. A questionnaire is considered to be internally consistent when there is a high correlation among the scores of items. This inter correlation is expressed by Cronbach's alpha. The minimum requirement for an instrument to be internally consistent is a value of 0.7016. Several other investigators have assessed the internal reliability of the DLQI, and have demonstrated Cronbach's Alpha scores of between 0.75 and 0.9217. Among patients with neurodermatitis, Cronbach's alpha was 0.889, indicating high internal reliability. Skin of eyelids, lips, hands and feet is frequently affected. In contrast to the Vidal’s disease, it develops mainly in childhood, often combined with other manifestations of atopy that affords grounds for consideration the disease as atopic neurodermatitis in these cases. Sometimes cataract (Andogsky syndrome), often ordinary ichthyosis are detected. In children, skin lesion of the type of eczema neurodermatitis may be a manifestation of Wiskott-Aldrich syndrome, an inherited recessive, coherent with X-chromosome and in addition, manifesting by thrombocytopenia, hemorrhagic diathesis, dysglobulinemia, an increased risk of development of infectious and malignant diseases, first of all, of lymphatic histiocytic system. Bacterial complications are represented by folliculitis, furunculosis, impetigo, and hydradenitis. The causative agent of these complications is usually Staphylococcus aureus, less epidermal one, white staphylococci or streptococci, the source of which are centers of chronic infection. The development of complications is accompanied by chill, increase of body temperature, sweating, increased congestion and itching. Peripheral lymph nodes are enlarged, painless. Breaking the itch-scratch cycle of neurodermatitis is challenging. Treatment success depends on resisting the urge to rub or scratch the affected areas. Over-the-counter and prescription creams can help ease the itching. You'll also need to identify and eliminate factors that may be aggravating the problem.
