The skin may become thickened and hyperpigmented (lichenified) as a direct result of chronic exoriation.[3] Typically this period of increased scratching is associated with stressors. There is a predilection for the nape of the neck, the ulnar border of the forearms, the wrists, the pretibial region, the dorsa of the feet, and the perianal and genital region. Atopic individuals are more prone than others to develop lichen simplex chronicus. Corticosteroid injections. Your doctor may inject corticosteroids directly into the affected skin to help it heal. A thick layer of compact orthokeratosis (resembling that seen on normal palms and soles) is present, overlying hypergranulosis. Focal zones of parakeratosis are sometimes interspersed with the orthokeratosis, but there is not the confluent parakeratosis of psoriasis. The epidermis shows psoriasiform hyperplasia with thicker rete ridges of less even length than in psoriasis. Epidermal thickness and volume are greater than in psoriasis. Minimal papillomatosis is sometimes present in a few areas. Focal excoriation is another change that may be seen in lichen simplex chronicus.
