From atopic dermatitis to asthma: the atopic march. Ann Allergy Asthma Immunol. 2010 Aug. 105(2):99-106; quiz 107-9, 117. [Medline]. Atopic dermatitis (AD) is associated with colonization and increased risk of infections with Staphylococcus aureus. However, it's unclear how the skin microbiome during infancy affects the incidence of AD. This prospective study evaluated the effects of skin microbiome on the risk of developing AD during the first year of life. The study included 50 randomly selected infants from an Irish birth cohort study. Skin swabs from four sites relevant to AD were collected at 2 days and 2 and 6 months of age, with bacterial 16S rRNA gene sequencing and analysis performed directly from clinical samples. Patterns and changes in bacterial skin colonization were analyzed for association with the incidence of AD at 1 year. The types and diversity of bacteria in the skin microbiome changed significantly between sampling periods. In contrast to patients with established AD, occurrence of infantile AD was not associated with skin dysbiosis or colonization with S. aureus. Development of AD during the first year of life was associated with significant differences in bacterial communities detected in swabs from the antecubital fossa at 2 months. In particular, commensal staphylococci were significantly less abundant in infants who went on to develop AD. The skin microbiome was unrelated to mode of delivery or feeding method. Skin colonization with S. aureus does not appear to occur before the development of AD in infants. Early colonization with commensal staphylococci may have a protective effect against infantile AD. The authors emphasize the need for further research to understand the pathophysiology and mechanisms by which the skin microbiome affects the development of skin immunity and AD. The eczema appears on typical areas of skin, depending on the sufferer’s age. During infancy it mainly affects the face, the upper body, hands and extensor surfaces of the arms and legs. In small children, the episodes mainly occur in the hollows of the knees, elbows, on the face, neck and throat. Hand and foot eczema may additionally be observed in older children, adolescents and adults. Videos Diseases & Conditions What do need to examine? Two premature babies and one child with multiple anomalies died; one newborn had a unilateral cystic kidney without urine production. In a retrospective analysis of 100 pregnancies with 68 live-born children, there were four with different malformations (Kainz 2000). Retrospective cases report premature twins of 32 weeks’ gestation. Both suffered from cardiomyopathy, of which one of them died (Vyas 1999). Among nine tacrolimus-exposed pregnancies, there were two with a small ventricular septal defect (Nagy 2003). An otherwise healthy premature baby who was exposed in utero to tacrolimus and mycophenolate mofetil demonstrated hypoplastic fingernails and short fifth fingers on both hands (Pergola 2001). There are rare case reports on the use of tacrolimus during pregnancy for other indications, such as refractory inflammatory bowel disease (Baumgart 2005). See Clinical Presentation for more detail. GET INVOLVED Deals and Healthy Teens Vidal’s disease (syn.: lichen simplex chronicus Vidal, dermatitis lichenoides pruriens Neisser) is clinically manifested by one or more strongly dry itchy plaques, located mainly on the posterolateral surfaces of the neck, in the folds of skin and surrounded by small papular elements and minor pigmentation, mildly turning into normal skin. Sometimes in the areas of scratching depigmentation develops. In the presence of pronounced infiltration and lichenification, hypertrophic warty foci of lesions can occur. Rare variants include depigmented, linear, moniliform, decalvans, psoriasiform neurodermatitis, and Pautrier’s giant lichenification. 2.
