Severe itching is a prominent feature of neurodermatitis, however psoriasis patients suffer from intensive itching are rarely concerned. Consistent with our finding, Szepietowski JC found that itching was found in 80% of psoriatic patients recently11. The presence and intensity of itching did not depend on age, gender, type of psoriasis, and duration of disease. Current evidence has shown that nonlesional skin is not normal skin, owing to persistent subclinical inflammation throughout the body.1-4 Chronic skin inflammation can be orchestrated by a mixture of activated immune and epidermal cells and can lead to pruritic lesion development.5-11 A growing consensus sees the epidermal layer of skin in atopic dermatitis as primed to react to environmental triggers, with signs and symptoms resulting from underlying hyperactivity in the deeper layers.2 Editorial Policy Schmitt J, Chen CM, Apfelbacher C, et al. Take Action Awareness Month Bacterial infection with S aureus or Streptococcus pyogenes is not infrequent in the setting of AD. The skin of patients with AD is colonized by S aureus. Colonization does not imply clinical infection, and physicians should only treat patients with clinical infection. The emergence of methicillin-resistant S aureus (MRSA) may prove to be a problem in the future in these patients. Eczematous and bullous lesions on the palms and soles are often infected with beta-hemolytic group A Streptococcus. LICHEN SIMPLEX CHRONICUS In severe disease, methotrexate, azathioprine, cyclosporine, and mycophenolate mofetil [4] Aktuelle Fortbildungen ABSTRACT: Neurodermatitis is a chronic disease affecting the patient's psychosocial status and quality of life. It is associated with a variety of psychologic problems, including demoralization, depression, anxiety, obsessive‐compulsive disorder, and sleep disturbances. Coexistence of sexual dysfunction, especially in women, with several systemic diseases has gained interest in recent years. In this study, we evaluated sexual function in female patients with neurodermatitis. We enrolled 89 women (43 patients, 46 controls) in the study. Quality of life was assessed with the Dermatology Life Quality Index (DLQI), and the Female Sexual Function Index (FSFI) was used to determine sexual function. Individuals with psychiatric disorders and/or those using antidepressants were excluded. The total DLQI score was 11.95 ± 5.65 in patients with neurodermatitis. The total FSFI score was significantly lower in patients compared with healthy controls (22.76 ± 5.31 and 28.83 ± 3.50, respectively; P = .001). Domain scores of FSFI (desire, arousal, lubrication, orgasm, and satisfaction) except pain were significantly lower in patients with neurodermatitis (P = .001). The pain score was also lower in patients than controls, but the difference was not statistically significant (P = .073). Neurodermatitis may be associated with sexual dysfunction, and patients with neurodermatitis should be evaluated with regard to sexual function to provide a better quality of life. When this is not the case, eczema is usually symmetrical and does not extend beyond the areola (see Fig. 12.6). The possibility of an artefactual syndrome (see Ch. 17) should not be overlooked.
