Atopic dermatitis (AD) (or atopic eczema) is a chronic skin condition that affects the quality of life of both adults and children. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety. Who to contact? It's hard to name a disease, in which the exact implementation of all preventive and therapeutic recommendations would be more important than in the presence of neurodermatitis. Furthermore, it should be emphasized that it must not be treated by waiting ("it would pass with age") and mask (a prescription of only antihistamines and hormonal ointments) treatments. Though a lot more has been learned about immunopathogenesis of AD, recognition of itching and scratching as hallmarks of this disease remains true. Itching is often worse at night. Chronic sleep disturbance due to frequent scratching can have adverse effects on the patient and his/her immediate family members and is a source of significant stress. When subjects with AD get upset, they tend to itch even more, probably secondary to flushing of the skin due to vasodilatation induced by neurogenic peptides, followed by histamine and prostaglandin E2 (Ostlere, Cowen et al. 1995). Papules, lichenification, and eczematous lesions result from repeated skin trauma. Vesicles and erythema with excoriation of the skin are seen in acute eczema. Crusted lesions and pustules are seen in infected eczema. Allergens, irritants (wool, soap, detergents, heat and humidity with sweating), infections, and certain foods can worsen eczema, and such history helps establish the right diagnosis. Distribution of rash on the face and extensor surfaces in infants and young children, changing to mostly flexural involvement in older ages, is a classic finding of AD and one of the essential features required to make this diagnosis. Dry skin (xerosis), personal and family history of allergic diseases, palmar hyperlinearity, nipple eczema, kera-tosis pilaris, pityriasis alba, and non-specific hand or foot dermatitis are some of the other clinical manifestations of this condition (Leung and Bieber 2003; Leung, Jain et al. 2003). The impact of AD on self-esteem and social interactions of kids or adults with this condition cannot be underestimated (Lapidus and Kerr 2001). Clinically, lesions appear erythematous or hyperpigmented, lichenified and scaly. High-potency topical steroids are often required to break the cycle. Steroid-impregnated tape, such as flurandrenolide (Cordran), applied at bedtime or after bathing and left in place for up to12 hours, also protects the lesions from being scratched. When symptoms improve, the potency of the steroid and the frequency of use can be reduced. Topical doxepin relieves pruritus and also helps break the cycle, but systemic absorption and drowsiness sometimes limit its use. If applicable, lesions can be covered with dressings, such as an Unna boot, to prevent the patient from scratching. More nodular lesions are termed prurigo nodularis, or picker’s nodules. Journal Info Your skin becomes painful or looks infected Board of Directors In the adult population, the rate of AD frequency is 3% or higher, but onset may be delayed until adulthood. Approximately two-thirds of the patients were followed for at least 2 years and the rest were followed for at least 5 years. From ages 2 to 26 years, more than 80% of patients reported having continued symptoms and/or use of topical medications to control symptoms. By age 20, approximately half of the patients had experienced at least one 6-month symptom- and medication-free period. The impact of neurodermatitis on sexual function is of interest because the disease can limit a person's ability to perform essential life tasks and social roles. Moreover, sexual dysfunction may also cause significant personal distress and negatively impact quality of life (Thaipisuttikul, 1998; Abeck, 2005; Chuh et al, 2006; Burgin, 2008). In our study, we used the DLQI to evaluate the quality of life of patients. The mean DLQI score of 11.95 is similar to the mean scores of 11.20 and 10.53 reported for atopic dermatitis and psoriasis, respectively (Basra et al, 2008). The scores on question 9 of the DLQI showed that neurodermatitis affected sexual function in our patients. Consistent with this observation, previous studies have revealed a higher prevalence of sexual dysfunction in patients with chronic pain (Monga et al, 1998; Ambler et al, 2001). Sexual dysfunction in female patients with fibromyalgia has been demonstrated (Tikiz et al, 2005), and sexual status and erectile dysfunction in patients with chronic obstructive pulmonary disease have been suggested (Koseoglu et al, 2005). Sexual dysfunction studies on other systemic diseases, such as diabetes mellitus, chronic liver disease, allergic rhinoconjunctivitis, epilepsy, chronic renal failure, and multiple sclerosis have also been performed (Ermertcan, 2009). Neurodermatitis, also known as lichen simplex chronicus, is a common chronic skin disease, affecting up to 12% of the total population, and women are more affected than men1. The disease is characterized by lichenificated plaque as a result of excessive scratching. Neck, elbow, ankles, vulva, eyelid even faces are the most common affected sites. Although neurodermatitis is not life-threatening, it can produce an important psychosocial burden. It has been suggested that patients with neurodermatitis suffer from depression, anxiety and other treatable psychological disorders2. Negative emotional states are the main personality component of patients (greater tendency to pain avoidance, greater dependency on other peoples' desires, and more conforming and dutiful)3. Patients with neurodermatitis tend to have poor social skills or interpersonal resources and a lack of flexibility. Neurodermatitis may be associated with sleep disturbance and sexual dysfunction4. All these data constitute a growing body of evidence indicating a negative impact of neurodermatitis on patients' quality of life (QoL). Acknowledgements Lichen simplex chronicus, also known as neurodermatitis circumscripta, is a very common chronic skin disease (present in up to 12% of the population) that is more often seen in women (Thaipisuttikul, 1998; Burgin, 2008; Stander and Metze, 2009). It is associated with a variety of psychologic problems, including demoralization, depression, anxiety, obsessive‐compulsive disorder, and sleep disturbances (Burgin, 2008). It may also have negative effects on the patient's quality of life (Abeck, 2005; Chuh et al, 2006). There are few reports about sexual function disorders associated with skin diseases (Türel Ermertcan et al, 2006; Ergun et al, 2007; Sukan and Maner, 2007; Mercan et al, 2008; Ermertcan, 2009). In this study, we compared the sexual function in female patients with neurodermatitis with that in healthy controls. Eczema is a skin condition characterised by an itchy, red rash, which affects 5% to 20% of people worldwide. There is no cure, but many treatments can help improve the skin's condition, making life easier. In those for whom these treatments do not work well or who fear their long‐term effects, there is often a belief that either something in their diet, or something missing in their diet, is making their eczema worse.
