It is believed that the consequence of cAMP decreased level can be an increased release of inflammatory mediators from leukocytes, including histamine, which causes decrease in the functional activity of T-lymphocytes via H2-receptor. This may explain IgE hyperproduction. The association with certain antibodies of tissue histocompatibility (HLA-A1, A9, B12, D24, DR1, DR7 and others) is found. According to the data of P. M. Alyeva (1993) DR5 antigen is a risk factor of disease development, and DR4 and DRw6 antigens are resistance factors. Most authors consider Vidal’s disease and disseminated neurodermatitis as an independent disease, but in patients with Vidal’s disease, the detection of immune phenomena, which are peculiar to atopic dermatitis, difference absence in the distribution of histocompatibility antigens in patients with the various prevalence of the process, the similarity in the disturbance of the metabolism of biogenic amines allow to consider disseminated neurodermatitis and Vidal’s disease as manifestations of one disease. Neurodermatitis around the anklebone: Scratching an itchy patch around the ankle caused the neurodermatitis to appear. Nummular eczema (Medical Encyclopedia) Also in Spanish Vidal’s disease (syn.: lichen simplex chronicus Vidal, dermatitis lichenoides pruriens Neisser) is clinically manifested by one or more strongly dry itchy plaques, located mainly on the posterolateral surfaces of the neck, in the folds of skin and surrounded by small papular elements and minor pigmentation, mildly turning into normal skin. Sometimes in the areas of scratching depigmentation develops. In the presence of pronounced infiltration and lichenification, hypertrophic warty foci of lesions can occur. Rare variants include depigmented, linear, moniliform, decalvans, psoriasiform neurodermatitis, and Pautrier’s giant lichenification. PORTUGUÊS The disturbance of microvessel tone is combined with a change in the rheological properties of skin that leads to a disruption of structural and barrier functions of skin and mucous membranes, increasing their permeability to antigens of different nature and promotes development of infectious complications. Immune disorders lead to polyvalent sensitization that is a basis of an underlying atopy (unco disease), which refers to the increased sensitivity of an organism to various stimuli. Therefore, these patients are often observed with the combination of neurodermatitis and other atopic, mainly respiratory, diseases, such as vasomotor rhinitis, bronchial asthma, hay fever, migraine and others. Coexistence of sexual dysfunction, especially female sexual dysfunction, with different diseases has gained interest in recent years (Cayan et al, 2004; Laumann et al, 2005; Bachmann, 2006; Hayes et al, 2006; Segraves and Woodard, 2006). Numerous studies have been performed in other disciplines of medicine about sexual dysfunction (Monga et al, 1998; Ambler et al, 2001; Koseoglu et al, 2005; Tikiz et al, 2005). J Am Acad Dermatol. 2001; 44: S58-S64 For Researchers Text a Donation Patients with atopic dermatitis need safe and effective long-term treatments. Previous studies have suggested benefits with dupilumab: a human monoclonal antibody against interleukin (IL)-4 receptor alpha that inhibits signaling of the type 2 cytokines IL-4 and IL-13. Two randomized, placebo-controlled trials of dupilumab for AD are reported. The SOLO 1 and 2 trials included 671 and 708 patients, respectively, with moderate to severe AD that was inadequately controlled by topical medications. Patients were assigned to 16 weeks of treatment with dupilumab, 300 mg given weekly or alternating with placebo every other week; or placebo given weekly. The primary outcome was a score of 0 or 1 on the Investigator's Global Assessment, indicating clear or almost clear of AD; plus at least a 2-point reduction in the same score from baseline to 16 weeks. The primary outcome was achieved in 37% of patients receiving weekly dupilumab and 38% with dupilumab every other week, compared to 10% with weekly placebo. Results were similar across the two trials. The dupilumab groups were also more likely to achieve at least 75% improvement in the Eczema Area and Severity Index. Other key outcomes were also improved with dupilumab, including pruritus, anxiety and depression symptoms, and quality of life. The main adverse effects of dupilumab were injection site reactions and conjunctivitis. The SOLO 1 and 2 results show significant improvement in AD signs and symptoms with dupilumab over 16 weeks. The benefits appear similar with treatment given weekly or every other week, compared to placebo. Further studies are needed to establish dupilumab's longterm safety and effectiveness. Quality Computing Services 2 Causes Colorado The Lancet Neurology by Dr. Miriam Kinai (Author) The impact of neurodermatitis on sexual function is of interest because the disease can limit a person's ability to perform essential life tasks and social roles. Moreover, sexual dysfunction may also cause significant personal distress and negatively impact quality of life (Thaipisuttikul, 1998; Abeck, 2005; Chuh et al, 2006; Burgin, 2008). In our study, we used the DLQI to evaluate the quality of life of patients. The mean DLQI score of 11.95 is similar to the mean scores of 11.20 and 10.53 reported for atopic dermatitis and psoriasis, respectively (Basra et al, 2008). The scores on question 9 of the DLQI showed that neurodermatitis affected sexual function in our patients. Consistent with this observation, previous studies have revealed a higher prevalence of sexual dysfunction in patients with chronic pain (Monga et al, 1998; Ambler et al, 2001). Sexual dysfunction in female patients with fibromyalgia has been demonstrated (Tikiz et al, 2005), and sexual status and erectile dysfunction in patients with chronic obstructive pulmonary disease have been suggested (Koseoglu et al, 2005). Sexual dysfunction studies on other systemic diseases, such as diabetes mellitus, chronic liver disease, allergic rhinoconjunctivitis, epilepsy, chronic renal failure, and multiple sclerosis have also been performed (Ermertcan, 2009). Arch Dermatol. 1999; 135: 583-586 Atopic dermatitis (AD) (or atopic eczema) is a chronic skin condition that affects the quality of life of both adults and children. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety.
