Images in this article That itching feeling that is normal for dry patches of skin may have no effect on some people and a chronic effect on others. The incidence of neurodermatitis, which is also known as atopic dermatitis or atopic eczema, and of psoriasis have increased in the last ten to fifteen years. Neurodermatitis is characterized by chronic itching or scaling of the skin. It starts with a patch of itchy skin, which leads to scratching. Scratching makes the affected area even itchier and the sufferer scratches a little more until he or she scratches out of habit. Chronic itching and scratching can cause the affected area to become thick and leathery. It is therefore essential to break this itch-scratch cycle. However, it's unclear how the skin microbiome during infancy affects the incidence of AD. This prospective study evaluated the effects of skin microbiome on the risk of developing AD during the first year of life. The study included 50 randomly selected infants from an Irish birth cohort study. Skin swabs from four sites relevant to AD were collected at 2 days and 2 and 6 months of age, with bacterial 16S rRNA gene sequencing and analysis performed directly from clinical samples. Patterns and changes in bacterial skin colonization were analyzed for association with the incidence of AD at 1 year. The types and diversity of bacteria in the skin microbiome changed significantly between sampling periods. In contrast to patients with established AD, occurrence of infantile AD was not associated with skin dysbiosis or colonization with S. aureus. Development of AD during the first year of life was associated with significant differences in bacterial communities detected in swabs from the antecubital fossa at 2 months. In particular, commensal staphylococci were significantly less abundant in infants who went on to develop AD. The skin microbiome was unrelated to mode of delivery or feeding method. Skin colonization with S. aureus does not appear to occur before the development of AD in infants. Early colonization with commensal staphylococci may have a protective effect against infantile AD. The authors emphasize the need for further research to understand the pathophysiology and mechanisms by which the skin microbiome affects the development of skin immunity and AD. Atopic dermatitis (AD) (or atopic eczema) is a chronic skin condition that affects the quality of life of both adults and children. Topical corticosteroids (TCS) are the main ointments used for treatment, but there is a risk of side‐effects with their use, such as skin thinning. A class of drugs called topical calcineurin inhibitors, which include topical tacrolimus (and pimecrolimus), might provide an alternative to this problem, but since tacrolimus is a newer ointment compared with corticosteroids, there are still some questions about its effectiveness and safety. Whether the primary immune dysregulation causes secondary epithelial barrier breakdown or primary epithelial barrier breakdown causes secondary immune dysregulation that results in disease remains unknown. However, given the fact that filaggrin is critical for epithelial integrity, it is now thought that loss of filaggrin function leads to increased transepidermal penetration of environmental allergens, increasing inflammation and sensitivity and potentially leading to the atopic march. Ubhi B Neill and Ridley (2001) reported that anogenital lichen sclerosus may commonly cause sexual dysfunction. Other vulvar dermatoses such as psoriasis, seborrheic dermatitis, squamous cell hyperplasia, lichen simplex chronicus, contact dermatitis, and erosive lichen planus can result in vulvodynia, dyspareunia, and sexual dysfunction (Davis and Hutchison, 1999; Burrows et al, 2008). Because anogenital skin lesions may result in difficulties in sexual function, we did not include patients with anogenital neurodermatitis in our study.
