Genome Data Viewer (GDV) Because there are a number of other lichenoid eruptions that resemble lichen planus a biopsy confirmation is frequently necessary. Persistent oral or vaginal lichen planus, with spots that thicken and grow together, can sometimes be difficult to distinguish clinically from whitish precancerous plaques called leukoplakia. A biopsy can be helpful in this situation. There seem to be a few patients in whom ulcerative lichen planus precedes the development of oral cancer. Mucous membranes: Lichen planus of the mucous membranes of the mouth and genitals is common. It appears as a white, lacy rash on the inside of the cheeks or the vagina and may not cause symptoms. The oral rash often occurs prior to any skin involvement. More troublesome, although rare, is erosive lichen planus, which can be quite sore and uncomfortable. This erosive form typically causes the patient to complain of the shallow and often quite painful, recurrent ulcers in the mouth or genitals. Lichen planus of the female genitalia can involve the external vulvar mucosa and the internal vaginal mucosa. There is an association with cancer in these areas, so both the oral and vaginal tissues need to monitored by a medical professional. To stay free, we rely on revenue from ads. In contrast to cutaneous LP, which is self limited, lichen planus lesions in the mouth may persist for many years,[43] and tend to be difficult to treat, with relapses being common.[34] Atrophic/erosive lichen planus is associated with a small risk of cancerous transformation,[43] and so people with OLP tend to be monitored closely over time to detect any potential change early. Sometimes OLP can become secondarily infected with Candida organisms.[citation needed] Dermatologic drug shortages Lichenin Springer Nature remains neutral with regard to jurisdictional claims in published maps and institutional affiliations. LICHEN PLANUS IN HET MONDSLIJMVLIES Endocrine and Metabolic Disorders + Postirradiation hyperkeratoses + Blistering diseases A 69-year-old man presented to the dermatology clinic with a 2-month history of a pruritic rash. The rash had first appeared on his right wrist and within 2 weeks had spread to his arms, legs, and trunk. His medical history was notable for chronic hepatitis C virus (HCV) infection, for which he had completed a 12-week course of elbasvir and grazoprevir 3 months before the onset of the rash. On examination, numerous purple papules were noted on the anterior surface of the forearms (Panel A) and the dorsal surface of the hands (Panel B) as well as on the trunk and legs. Several lesions showed a fine reticulate pattern of dots and lines, called Wickham’s striae. Biopsy of a lesion revealed hyperkeratosis, wedge-shaped hypergranulosis, sawtooth rete pegs, and bandlike lymphocytic inflammation — findings confirming the diagnosis of lichen planus. Although the cause of lichen planus has not been established definitively, it is thought to be an autoimmune disease. Lichen planus has a known association with HCV infection. In this patient, the association between HCV infection, which had been treated, and the later eruption was unclear. Topical or intralesional glucocorticoids may be used in primary treatment, with ultraviolet light therapy and oral immunosuppressive agents reserved for more extensive cases. This patient was treated with topical clobetasol and narrow-band ultraviolet B therapy, and a reduction in lesions occurred within 1 month after the start of treatment. lichen planus | sunburn lichen planus | eczema treatment lichen planus | psoriasis treatment
