Mouth Sores Oral lichen planus (OLP) is a chronic immune-mediated disease that affects the oral mucosa. The prevalence rate of OLP is reported as 0.5–2.2% [1, 2]. A recent systematic review indicated that the malignant transformation rate of OLP-affected tissue is 1.09% [3]. There are several types of OLP, the most common of which are the reticular, atrophic, and ulcerative/erosive forms. Often, the clinical appearance of OLP mimics other types of vesiculobullous diseases; therefore, biopsy of the lesion is required for definitive diagnosis [4]. Histopathological features of OLP include hyperkeratosis, acanthosis, liquefactive degeneration of the basal cell layer, and prominent lymphocyte infiltration at the epithelium-connective tissue interface [1]. Use of direct immunofluorescent (DIF) staining also aids in the diagnosis of OLP. Recently, it was revealed that the most common manifestations found on DIF analysis of OLP biopsies are 1) shaggy expression of fibrinogen in the basement membrane zone; 2) deposition of Immunoglobulin M as colloid bodies; 3) deposition of C3 in granular and linear patterns. Thus, the presence of these proteins on DIF analysis is utilized for diagnosis of OLP [5]. Background  Oral lichen planus (OLP) is a chronic inflammatory disorder that can cause local irritation and discomfort with attendant poor dentition and nutrition. Although a range of therapeutic options is available, data on the long-term efficacy of treatments for this chronic disease are limited. To identify agents that might be effective in OLP treatment over a longer term, and to explore their sequential use in treatment-refractory patients, we studied patients who received multiple OLP therapies and who were followed up for an average of more than 2 years. Hair Involvement Table 6 + Psoriasis Candida species are normally harmless yeasts found in the mouth, intestinal tract, and vagina. Candidiasis (also known as a yeast infection or thrush) may occur when an imbalance is created in the host’s body and usually affects the skin and/or the mucous membranes of the mouth, intestines, or the vagina. Oral candidiasis can look similar to oral LP, with white patches in the mouth. Candida infections are rarely serious in otherwise healthy people. In people with oral LP, superficial Candida infections may also occur in combination with LP, especially in people using topical steroids. Both pemphigus and pemphigoid occur as solitary erythematous lesions and are not associated with any white striae. This can aid in clinical differential diagnosis as erosive and atrophic forms of OLP usually show concomitant reticular form. Peeling of the epithelium from the epithelium–connective tissue junction on slight lateral pressure in nonaffected area (Nikolsky's sign) differentiates it from erosive and erythematous forms of lichen planus. A biopsy from the perilesional tissue can diagnose pemphigus or pemphigoid, which show intraepithelial or subepithelial split histologically. In some cases, erythema multiforme (EM) can resemble bullous lichen planus, but EM is more acute and generally involves the labial mucosa. Chronic ulcerative stomatitis (CUS) is an immune-mediated disorder affecting the oral mucosa which clinically and histopathologically resembles lichen planus. Diagnosis of CUS is based on direct immunofluorescence studies where autoantibodies are directed against p63 in the basal and parabasal layers of the epithelium. These lesions have to be differentiated from lichen planus because CUS does not respond to corticosteroid therapy and has to be treated using antimalarial drugs.[12] Variants of cutaneous lichen planus are distinguished based upon the appearance of the lesions and/or their distribution.[18] Lesions can affect the: Erosive LP Fairly common in oral LP. New areas should be evaluated for conversion to squamous cell carcinoma (SCC). Lesional biopsies should be performed with any non-healing erosion. 7 Any mouth disease can lead to gum disease.
