The psychological impact of vitiligo is profound in childhood. Associated negative experiences can include fear of being questioned about one’s appearance, teasing and bullying, anxiety over the potential for disease spread, interference with emotional maturation, depression, and interference with socialization (eg, sexual debut). Of the 25% of children with vitiligo evaluated for psychological issues, 60% reported psychological problems in a cohort of 119 Brazilian children.16 A recent review article of quality-of-life issues in childhood skin disease stated: “In general, patients with vitiligo experience low self-esteem, social stigmatization, shame, avoidance of intimacy, anxiety, depression, adjustment disorder, fear, suicidal ideation, and other psychiatric morbidity.”47 Recent studies have suggested that childhood vitiligo impacts quality of life similarly to psoriasis, as children enter adulthood.

Vitiligo is an acquired depigmenting disorder that affects 0.5% to 2% of the world population. Three different forms are classified according to the distribution of lesions; namely non-segmental, segmental and mixed vitiligo. Vitiligo is associated with polymorphisms in genes involved in the immune response and in melanogenesis. However, environmental factors are required for the development of manifest disease. In general, the diagnosis is clinical and no laboratory tests or biopsies are required. Metabolic alterations are central to current concepts in pathophysiology. They induce an increased generation of reactive oxygen species and susceptibility to mild exogenous stimuli in the epidermis. This produces a senescent phenotype of skin cells, leads to the release of innate immune molecules, which trigger autoimmunity, and ultimately causes dysfunction and death of melanocytes. Clinical management aims to halt depigmentation, and to either repigment or depigment the skin, depending on the extent of disease. New therapeutic approaches include stimulation of melanocyte differentiation and proliferation through α-melanocyte-stimulating hormone analogues and through epidermal stem cell engineering. Several questions remain unsolved, including the connection between melanocyte depletion and stem cell exhaustion, the underlying degenerative mechanisms and the biological mediators of cell death. Overall, vitiligo is an excellent model for studying degenerative and autoimmune processes and for testing novel approaches in regenerative medicine. Sometimes, the skin suddenly stops producing melanin. At first, this might cause a small spot, called a macule, that’s lighter in color than the skin around it. In time these white patches may spread and grow to cover a larger portion of the body. Sometimes these white patches spread quickly at first and then remain stable for years. Other times the spread is slower, occurring over a longer period of time.

SV occurs focally, based on localized susceptibility to disease and is not associated with autoimmune phenomena unless nonsegmental disease or a generalized autoimmune condition such as alopecia areata occurs concurrently.31,32 The leading theory is that generalized vitiligo is a multifactorial, polygenic autoimmune disorder that occurs in only a minority of genetically susceptible individuals and is therefore believed to have a strong component of environmental triggering. Other theories of vitiligo development include biochemical defects in the tetrahydrobiopterin pathway/oxidative damage, adhesion defects, and neural induction33; however, generalized disease at this point in time is usually presumed autoimmune in nature, with demonstrable autoantibodies against pigment cells in patients with vitiligo.34 In a survey of 2624 primarily White probands, frequency was 6.1% in siblings, and concordance only 23% in identical twins. Early-onset disease was associated with more family members, suggesting that this subgroup carries more genetic susceptibility determinants.35 Associated autoimmune diseases in family members in this cohort were vitiligo itself, autoimmune thyroid disease (particularly hypothyroidism), pernicious anemia, Addison disease, systemic lupus erythematosus, and inflammatory bowel disease. Other studies have demonstrated personal and familial association of vitiligo with canity,21 atopic dermatitis,36,37 rheumatoid arthritis, types 1 and 2 diabetes mellitus, alopecia areata, psoriasis, chronic urticaria, lichen sclerosis et atrophicus, Celiac disease, systemic lupus erythematosus, and sarcoidosis.38 Extensive disease and increasing years with disease are the factors most associated with vitiligo-associated autoimmune disease.38

Cutaneous lichen planus may resolve spontaneously within one to two years, although lichen planus affecting mucous membranes may be more persistent and resistant to treatment. Recurrences are common, even with treatment. Table 4 summarizes the treatment of nongenital cutaneous lichen planus lesions. High-potency topical corticosteroids are first-line therapy for cutaneous lichen planus.14–16 Oral antihistamines (e.g., hydroxyzine [Vistaril]) may be used to control pruritus. Hypertrophic lesions are treated with intralesional triamcinolone acetonide (Kenalog), 5 to 10 mg per mL injection (0.5 to 1 mL per 2-cm lesion).14

Zani et al report that even with antifungal treatments, there was no reduction in levels of Malassezia. [23] The contribution of Malassezia species to seborrheic dermatitis may come from its lipase activity—releasing inflammatory free fatty acids—and from its ability to activate the alternative complement pathway. Although some doctors warn their patients that any treatment for scalp seborrheic dermatitis may lose its effectiveness after three months of use, a study evaluating a zinc scalp treatment at six and 11 months showed no loss of effect.20 A study of the long-term use of ketoconazole shampoo for maintenance therapy also showed no degradation in effectiveness over time.21 What may appear to be a worsening of symptoms despite continued use of a treatment may actually represent the natural, varying course of the disease. Often, reassurance and maintenance of therapy are all that is needed, reserving changes in treatment for persistent worsening of symptoms. Patients with symptoms that do not respond to any of the therapies outlined above may benefit from systemic anti-inflammatories and should be referred to a dermatologist.

Neurologic and psychiatric conditions, such as Parkinson’s disease and depression These medicines can cause skin thinning with overuse, so doctors will only recommend them for short-term use. Supplementing these topical medicines with natural treatments can decrease side effects and may be better in the long term.
