Career planning The severity and duration of an attack of shingles can be significantly reduced by immediate treatment with antiviral drugs, which include acyclovir, valcyclovir, or famcyclovir. Antiviral drugs may also help stave off the painful after-effects of shingles known as postherpetic neuralgia. Other treatments for postherpetic neuralgia include steroids, antidepressants, anticonvulsants (including pregabalin and gabapentin enacarbil), and topical agents.  The varicella zoster virus vaccines Shingrix and Zostavax have been approved by the Food and Drug Administration for adults age 50 and older.  Researchers found that giving older adults the vaccine reduced the expected number of later cases of shingles by half.  And in people who still got the disease despite immunization, the severity and complications of shingles were dramatically reduced. The shingles vaccine is a preventive therapy and not a treatment for those who already have shingles or long-lasting nerve pain (postherpetic neuralgia).

In many people, it never reappears. For others, though, the virus flares up and causes shingles. People can get shingles flare-ups more than once — though that doesn’t usually happen.

Most children now are vaccinated against chickenpox. In theory, this will translate into fewer Americans who suffer from shingles in the future. However, there are variables, such as mild cases of chickenpox with symptoms that aren’t even noted, and the fact that we’ve now learned that the immunity offered from vaccines wanes over time. So if today’s children don’t receive the boosters, they could be at risk for chickenpox, and then shingles, in the future.

dr. Jan R. Mekkes. Dermatoloog, AMC, Amsterdam. Oral lichen planus is a inflammatory condition that affects the inside of the mouth.  Signs and symptoms include patches of fine white lines and dots most commonly in the inside of the cheeks, gums, and/or tongue. Most people with lichen planus experience no to few symptoms, others may have painful sores or ulcers in the mouth. Severe lichen planus slightly increases the risk for oral cancer. Oral lichen planus may occur alone or in combination with other skin forms of lichen planus.[1][2][3]

CORDRAN 2018 © American Academy of Oral Medicine Although lichen planus classically manifests as a cutaneous rash of purple, polygonal, pruritic papules and plaques (known as the 5 P’s), the disease can affect the scalp, nails, genitalia, eyes, oral cavity, and even the esophagus. Oral lichen planus typically presents as 1 of 3 major clinical forms: reticular, atrophic, or erosive. This patient had erosive lichen planus, the most severe form of the disease, which exhibits denuding and sloughing of the mucocutaneous surface, represented either by erythematous bullae and ulcers or by a desquamative gingivitis.1 The lesions are most commonly found on the buccal mucosa; however, the tongue and labial mucosa may be involved.

Although a range of therapeutic options is available, data on the long-term efficacy of treatments for this chronic disease are limited. Lichen planus is an inflammatory disease of unknown cause. There are cases of lichen planus-type rashes occurring as allergic reactions to medications for high blood pressure, heart disease and arthritis. In those cases, identifying and stopping the use of the drug helps clear up the condition within a few weeks. Some people with lichen planus can also have hepatitis C and your dermatologist may want to check you for this. Lichen planus affects men and women equally, and occurs most often in middle-aged adults.

Additional information Menopause One of the striking features of this patient population has been the ability to achieve responses in patients in whom multiple prior therapies failed. The acquisition and maintenance of response in the oral lesions was the result of the application of a graded approach to drug administration, in which the intensity of an individual therapy is varied to achieve lesion regression and at the same time limit oral and systemic toxicity. Eventually, most patients achieved outstanding responses (Table 1). The therapeutic ladder is illustrated in Table 4.

Pregnancy State policy Meetings & events Pityriasis versicolor usually clears up quickly with treatment. But the skin patches may stay discolored for weeks or months. To make them less noticeable, it’s important to use sunscreen to prevent the skin from tanning or burning.
Recurrence is almost universal after treatment because the causative organism is a normal skin inhabitant. Fastidious hygiene, regular use of zinc pyrithione soap, or once-monthly use of topical antifungal therapy lowers the likelihood of recurrence.

Besides the rash, seen here on the back, there may be mild itching. Frequent sites of infection include the neck, upper chest, and arm pit (axilla). The rash may be white (as seen here) to yellowish to golden brown in color. A tan can accentuate the difference in skin color.

Oral antifungals including ketoconazole or fluconazole in a single dose, or ketoconazole for seven days, or itraconazole can be used.[17][18] The single-dose regimens, or pulse therapy regimens, can be made more effective by having the patient exercise 1–2 hours after the dose, to induce sweating. The sweat is allowed to evaporate, and showering is delayed for a day, leaving a film of the medication on the skin.[19]

Tinea versicolor can return. When the air outdoors is warm and humid, the yeast can quickly grow out of control. Some people who live in a tropical climate may need to use a medicated cleanser year round to prevent the yeast from overgrowing. People who live in an area that becomes warm and moist each spring may see tinea versicolor return every year.

Obstetrics and Gynecology Katherine Smyth Atwood, PA-C Tinea versicolor is very common, and though it’s annoying, it’s fortunately not dangerous. “It’s a skin condition caused by a specific kind of yeast called malassezia,” New York City dermatologist Janet Prystowsky, M.D. tells SELF. Malassezia is something that exists naturally on the skin, and tinea versicolor is an overgrowth of this yeast. Malassezia is also the cause for other skin conditions like dandruff and psoriasis. And while it can’t be spread by skin-on-skin contact (meaning it’s not contagious), it’s hard to determine just what causes tinea versicolor, says Schultz.

Lesions appeared from 3 and 48 months after institution of therapy. At diagnosis, 84.6% of patients were on indinavir therapy. CD4 valus ranged from 120 and 1,332 cells/mm3 and viral load was lower than 200 copies/ml in 92.3 of cases. Conservative therapy was aplied in 7 patients and surgery in 6. In all patients indinavir therapy was discontinued, and cure was achieved 16 weeks later. The «retinoid» effect of indinavir is discussed as likely pathogenic explanation for this complications. We advocate for topic therapy and change of anti-retroviral therapy, reserving surgery for patients not responding to therapy. Pain and functional limitation caused by this non uncommon complication (1.6% of our patients treated with antirtroviral agents) makes its knowledge necessary and an active search by clinicians in patients receiving indinavir therapy.
