Systemic non-biological medication for psoriasis Biologic Agents offer hope to those who haven’t had success with other approaches or were uncomfortable with the potential side effects of older systemic medications. Biologic agents are administered by injection. Different from traditional systemic drugs that impact the entire immune system, biologics target specific parts of the immune system. They treat psoriasis by reducing inflammation in the skin and elsewhere in the body. These medicines are intended to treat patients with moderate to severe psoriasis and patients with psoriatic arthritis. Selection of a specific agent is based on the patient’s medical history, the presence or absence of psoriatic arthritis, and other factors. Biologic agents which we prescribe include:

Other studies2,14,23,38,39 performed on patient preferences for psoriasis treatments have compared selected treatment options. However, the available evidence on preferences for or satisfaction with the existing systemic therapies and phototherapies is surprisingly limited, and, in a recent systematic review,39 it was not possible to rank therapies according to patient preferences. For example, studies2,14,23,38 comparing preferences for classic systemic antipsoriatic therapies vs phototherapies have reported variable outcomes, with some studies showing preference for oral therapy and some for UV therapy. Patients who obtained biologicals appear to prefer these therapies over other treatments.38 Correspondingly, in a prospective study,40 patients with psoriatic arthritis achieved a rapid and sustained gain in health utility when receiving tumor necrosis factor antagonists.

Choice of specific agents and combinations requires close cooperation with the patient, always keeping in mind the untoward effects of the treatments. There is no single ideal combination or sequence of agents, but treatment should be kept as simple as possible. Monotherapy is preferred, but combination therapy is the norm. First-line treatment for psoriasis includes topical corticosteroids and topical vitamin D3 analogs (either as monotherapy or in combination).

Anthralin is available in 0.1 percent to 1 percent ointments, creams and solutions. It is generally used on notably thick, large plaques of psoriasis, and therapy is initiated at low concentrations for short periods. The concentration and duration of contact with each treatment is gradually increased, up to a maximum of 30 minutes per application.4 Anthralin can be combined with ultraviolet phototherapy; this is known as the traditional Ingram regimen.

When psoriasis is severe, doctors will likely use systemic treatment, which means prescription drugs or medicine given through a shot. Combination therapy for psoriasis uses topical, phototherapy and systemic treatments in unison, all of which come with side effects that a natural remedy like a psoriasis diet does not.

UVB phototherapy. Controlled doses of UVB light from an artificial light source may improve mild to moderate psoriasis symptoms. UVB phototherapy, also called broadband UVB, can be used to treat single patches, widespread psoriasis and psoriasis that resists topical treatments. Short-term side effects may include redness, itching and dry skin. Using a moisturizer may help decrease these side effects.

Sun exposure in addition to topical therapy may be beneficial when multiple areas are affected with psoriasis. Patients should be encouraged to obtain natural sunlight exposure or tanning-bed light exposure for a few minutes a day, and then to slowly increase the duration of exposure as tolerated. Unaffected areas should be covered with a sunscreen, especially the face. Ultraviolet light exposure can be used judiciously to prevent flare-ups during the maintenance phase of therapy.
