Drugs and Supplements A-Z Yoga, Pilates & Barre When stratifying participants according to the PASI and DLQI, patients with greater impairment of the quality of daily life (DLQI >10) attached greater importance to the probability of improvement compared with those with a lower DLQI, as expected. By contrast, the PASI was negatively correlated with participants’ preference for the probability of improvement. This finding may reflect the low correlation between objective clinical severity and psychological disability in psoriasis.43 However, the PASI measured in this study does not reflect the severity of psoriasis in these participants because many had achieved significant control with use of systemic medications. Finally, patients who place great value on the probability of improvement are likely to adhere to prescribed treatments, resulting in favorable outcomes shown in a low PASI.
About This Site Newer drugs are in development and no doubt will be available in the near future. As this class of drugs is fairly new, ongoing monitoring and adverse effect reporting continues and long-term safety continues to be monitored. Biologics are all comparatively expensive especially in view of the fact they none of them are curative. Recently, the FDA has attempted to address this problem by permitting the use of “biosimilar” drugs. These drugs are structurally identical to a specific biologic drug and are presumed to produce identical therapeutic responses in human beings to the original, but are produced using different methodology. Biosimilars ought to be available at some fraction of the cost of the original. If this will be an effective approach remains to be seen. The only biosimilar available currently is infliximab (Inflectra). Two other biosimilar drugs have been accepted by the FDA, an etanercept equivalent (Erelzi) and an adalimumab equivalent (Amjevita) — but currently, neither are available.

Surgical Dermatology Side effects of topical corticosteroids, especially those that carry the superpotent categorisation, include cutaneous atrophy, development of striae, formation of telangiectasia, and a host of other local cutaneous effects such as the formation of an acneiform eruption known as perioral dermatitis on the face.6,7 Hypothalamic–pituitary–adrenal (HPA) axis suppression can occur with prolonged use of excessive quantities of topical corticosteroids, particularly if they are occluded or if superpotent corticosteroids are used continuously over large areas of the body. However, the cutaneous side effects are more commonly problematic than significant HPA axis suppression, which is seldom an issue.8

Cerave Moisturizing Cream The use of biologic agents (proteins with pharmacologic activity) is discussed in Section 1 (2008) and reviewed, with updated safety information, in Section 6 of the AAD guidelines. The AAD recommends a set of baseline laboratory studies before starting treatment with a biologic agent to ensure any underlying conditions or risk factors are understood. [35, 38] Some patients with chronic hepatitis C may be safely treated with biologic agents, [46] while active hepatitis B is still considered a contraindication.

Immunosuppressants can be used for severe psoriasis. Cyclosporine is a commonly used immunosuppressant. It should be limited to courses of several months (rarely, up to 1 yr) and alternated with other therapies. Its effect on the kidneys and potential long-term effects on the immune system preclude more liberal use. Other immunosuppressants (eg, hydroxyurea, 6-thioguanine, mycophenolate mofetil) have narrow safety margins and are reserved for severe, recalcitrant psoriasis.

Osteopathic Manipulative Therapy All Proteins Resources… Climatotherapy, the oldest form of phototherapy involving exposure to sunlight, is well established at a number of clinics around the world. Perhaps the most successful is the psoriasis treatment centre at the Dead Sea.23 At 300 m below sea level, the Dead Sea is the lowest point on earth. Its mineral content is greater than that of any other naturally occurring body of water on earth. The extra 300 m through which sunlight has to pass, combined with the mineral haze over the Dead Sea, results in light exposure that has proved highly beneficial for psoriasis. Results are comparable with those obtained with broadband UVB phototherapy.24

Psoriatic plaques that fail to respond to topical therapy may be improved by administration of intralesional corticosteroid injections. Triamcinolone (Kenalog) is often used for this purpose. The agent is injected directly into the dermis of a small, persistent plaque. The concentration is generally 3 to 10 mg per mL, depending on the size, thickness and area of the lesion. The dose of triamcinolone is released gradually over three to four weeks; additional injections may be needed every four to six weeks to improve the response. Disadvantages of intralesional injections include pain during the injection and potential side effects of local atrophy and systemic absorption.

These treatments would be most appropriate for those with milder, plaque disease and not for those with pustules, erythroderma or arthritis.

Once the diagnosis of psoriasis is made, patient education about the disease should begin. Points that should be emphasized about the disease initially include its noncontagious nature and the possibility of controlling but not curing it. Patients should also be assured that psoriasis is quite common. Exacerbating factors should be discussed, including stress, infection, trauma, xerosis and use of medications such as angiotensin-converting enzyme inhibitors, beta-adrenergic blockers, lithium and the antimalarial agent hydroxychloroquine (Plaquenil).
