It occurs when melanocyte cells stop producing melanin, either because they die or because they stop functioning. Causes of vitiligo are unclear, but this skin condition is thought to be an autoimmune disease. It can appear at any age and on any part of the body. Sometimes, the white patches will spread throughout the body, while other times they will remain the same size. Vitiligo often appears initially as a small, pale spot on the skin that, over time, will develop into a larger patch.

The autoimmune theory proposes alteration in humoral and cellular immunity in the destruction of melanocytes of vitiligo. [9, 10, 11] This theory is of particular relevance given the fact that nonsegmental vitiligo is more frequently associated with autoimmune conditions than is segmental vitiligo. [12] Hence, making a diagnosis of nonsegmental vitiligo in a person with a family history of autoimmune disease may warrant a more thorough workup. For these reasons, certain disorders have been linked to vitiligo, such as: Hashimoto thyroiditis, Graves disease, Addison disease and diabetes mellitus, alopecia areata, pernicious anemia, inflammatory bowel disease, psoriasis, and autoimmune polyglandular syndrome. The most convincing evidence of an autoimmune pathogenesis is the presence of circulating antibodies against melanocyte proteins in patients with vitiligo. [13] In addition to humoral immune mechanisms, strong evidence indicates involvement of cellular immunity. Destruction of melanocytes may be directly mediated by autoreactive CD8 + T cells. Activated CD8 + T cells have been demonstrated in perilesional vitiligo skin. [10, 11]

An Internet-based survey of vitiligo probands ages 0 to 17 years and their families showed that pediatric vitiligo was associated with increasing disturbances in quality of life with advancing age. When asked whether they were bothered by their vitiligo, only 4.1% of teenagers 15 to 17 years were not bothered by their disease versus 45.6% of children ages 0 to 6 years and 50% of children 7 to 14 years. In particular, facial and leg involvements were most distressing to the patient. Facial and arm lesions were most associated with teasing and bullying.48 Similarly, Bilgiç et al49 demonstrated in a cohort of 41 children ages 8 to 18 years versus controls that vitiligo had a negative impact on the quality of life. Head and neck in boys and genitalia and legs in girls were correlated with negative impact on quality of life. Camouflage of visible facial lesions has been shown to improve quality of life in children with vitiligo.50 Depression and anxiety have been reported in 26% and 42% of parents/caregivers of children with pediatric vitiligo.51
    Vitiligo is not contagious. But King said people are often concerned when they see people with vitiligo on their hands. He said patients have told him that cashiers sometimes ask them to put money or credit cards down on the counter so they don’t have to touch their hands.

    Courtesy of Google, the only somewhat scientific explanation I later found was that altered pigmentation of the skin is usually brought about by our melanocytes destroying themselves within the comfort of our own bodies. For now, the doctor fidgeted with his ball point pen and spoke to us about topical creams, casually slipping in that there was currently no cure for the disorder — the last words I wanted to hear. Yet I was more concerned about the $300 check my parents were writing for the appointment, than my initial distaste with the doctor. During the moment when I needed the person with the degree to speak to me in a balance of scientific syntax and plain language, he was relatively mute and uneducated, as if this was the first time he spoke to someone “spotted.” I politely thanked him when the appointment was over and left the rubber-smelling cubicle.
