Related Focal spongiosis Seborrheic dermatitis is thought to be caused by a variety of factors. However, central to all these factors is the presence of a yeast known as Malassezia and some local or systemic alteration in the immune system. Malazzesia yeast are thought to help metabolize certain types of fats in the skin from non irritating to irritating and inflammatory fatty acids that cause inflammation. It’s clear that getting rid of these yeast helps control the symptoms and signs of seborrheic dermatitis. Most individuals with seborrheic dermatitis have a healthy immune system. However, it’s clear that there is some alteration in the skin immune system that prevents these individuals from eradicating the Malazzesia yeast.

Scaling of the skin on the scalp, eyebrows, forehead, ears, and beard area is typical. Some cases may also involve the central chest, underarms, groin, and around the belly button. In darker skinned patients, this scaling can have a flower-like shape (thus the term petaloid seborrheic dermatitis). Discoloration of the skin with lightening or darkening of affected areas may also occur. It is sometimes itchy. A dermatologist can determine if you have seborrheic dermatitis by clinical examination. In rare cases, a skin scraping or biopsy may be necessary to confirm the diagnosis or to rule out other explanations for your symptoms.

The mainstays of treatment for facial seborrheic dermatitis are topical antifungals, corticosteroids, and calcineurin inhibitors. Ketoconazole 2% is as effective as hydrocortisone 1% cream.22 Ketoconazole 2% gel (Xolegel) significantly reduced symptoms of erythema, pruritus, and scaling compared with vehicle alone.23 Ciclopirox appears to be better tolerated and more effective than ketoconazole 2% gel.24,25 Ciclopirox 1% cream (not available in the United States) was shown in a randomized, double-blinded trial to greatly reduce symptoms when used as a maintenance medication.24 Sertaconazole 2% cream (Ertaczo) was more effective than hydrocortisone 1% cream in one study.26 In light of their effectiveness, low adverse effect profile, and reasonable cost, topical antifungals are the preferred agents for acute and long-term treatment of seborrheic dermatitis of the face and body.3,14–16,22–26

  1. Skin biopsy may be helpful but is rarely indicated. Histological findings specific to seborrhoeic dermatitis are superficial perivascular and perifollicular inflammatory infiltrates, psoriasiform hyperplasia, and parakeratosis around follicular openings.

Malassezia organisms are probably not the cause of seborrheic dermatitis but a cofactor linked to a T-cell depression, increased sebum levels, and an activation of the alternative complement pathway. Various medications may also flare or induce seborrheic dermatitis. [2, 3]
