Background  Oral lichen planus (OLP) is a chronic inflammatory disorder that can cause local irritation and discomfort with attendant poor dentition and nutrition. Although a range of therapeutic options is available, data on the long-term efficacy of treatments for this chronic disease are limited. To identify agents that might be effective in OLP treatment over a longer term, and to explore their sequential use in treatment-refractory patients, we studied patients who received multiple OLP therapies and who were followed up for an average of more than 2 years.
Spicy foods, citrus juices, tomato products, caffeinated drinks like coffee and cola, and crispy foods like toast and corn chips can aggravate oral lichen planus, especially if there are open sores in the mouth.

Biopsy of a lesion revealed hyperkeratosis, wedge-shaped hypergranulosis, sawtooth rete pegs, and bandlike lymphocytic inflammation — findings confirming the diagnosis of lichen planus. Although the cause of lichen planus has not been established definitively, it is thought to be an autoimmune disease. Lichen planus has a known association with HCV infection. In this patient, the association between HCV infection, which had been treated, and the later eruption was unclear. Topical or intralesional glucocorticoids may be used in primary treatment, with ultraviolet light therapy and oral immunosuppressive agents reserved for more extensive cases. This patient was treated with topical clobetasol and narrow-band ultraviolet B therapy, and a reduction in lesions occurred within 1 month after the start of treatment.

    Few controlled studies have evaluated treatments. Options differ by location and extent of disease. Most cases of LP on the trunk or extremities can be treated with topical treatments. Topical corticosteroids are first-line treatment for most cases of localized disease. High-potency ointments or creams (eg, clobetasol, fluocinonide) may be used on the thicker lesions on the extremities; lower-potency drugs (eg, hydrocortisone, desonide) may be used on the face, groin, and axillae. As always, courses should be limited to reduce risk of corticosteroid atrophy. Potency may be enhanced with use of polyethylene wrapping or flurandrenolide tape. Intralesional corticosteroids (triamcinolone acetonide solution diluted with saline to 5 to 10 mg/mL) can be used every 4 wk for hyperkeratotic plaques, scalp lesions, and lesions resistant to other therapies.
    Advocacy priorities
    Current immunosuppressive therapies usually control oral mucosal erythema, ulceration, and symptoms in patients with oral lichen planus with minimal adverse effects. However, a range of therapies may need to be tried.
