When psoriasis is severe, doctors will likely use systemic treatment, which means prescription drugs or medicine given through a shot. Combination therapy for psoriasis uses topical, phototherapy and systemic treatments in unison, all of which come with side effects that a natural remedy like a psoriasis diet does not.

No specific strategies prevent psoriasis, although healthy lifestyles that avoid obesity and reduced alcohol use can make control easier and increase the chances of at least temporary remission. Whenever possible, patients who are currently being treated for psoriasis or have a history of psoriasis should avoid over-the-counter and prescription medications known to exacerbate it. This includes the use of over-the-counter NSAIDs such as ibuprofen and naproxen.

Methotrexate taken orally is an effective treatment for severe disabling psoriasis, especially severe psoriatic arthritis or widespread erythrodermic or pustular psoriasis unresponsive to topical agents or UV light therapy (narrowband UVB [NBUVB]) or psoralen plus ultraviolet A (PUVA). Methotrexate seems to interfere with the rapid proliferation of epidermal cells. Hematologic, renal, and hepatic function should be monitored. Dosage regimens vary, so only physicians experienced in its use for psoriasis should undertake methotrexate therapy.
Steroid-based creams. The mainstay of psoriasis treatment, steroid creams decrease inflammation, relieve itching, and block the production of cells that are overproduced in psoriasis. Stronger preparations, which are more effective than milder ones, can cause side effects that include burning, dryness, irritation, and thinning of the skin. Be especially careful to follow your doctor’s instructions on their use.

Psoriasis affects about 2 percent of the U.S. population. In 1990, psoriasis was the reason for more than 1 million visits to physicians.1 In the past, the treatment of psoriasis was primarily managed by dermatologists; this disorder accounted for only 0.4 percent of skin-related visits to family physicians.2 The number of patients treated by family physicians, however, is likely to have steadily increased as more patients seek medical care of psoriasis from their primary care physicians.

Once therapy is initiated, patients should be evaluated for appropriate treatment response at dedicated intervals. While the time to maximum therapeutic benefit depends on the agent of choice, European guidelines recommend that patients be evaluated after an induction phase (typically 16–24 weeks) and define treatment success as either (1) at least 75% improvement in PASI or (2) at least 50% improvement in PASI and a Dermatology Quality of Life Index (DLQI) score of 5 or lower.6

Some people say complementary and alternative medicine (CAM) helps them feel better. These treatments include special diets, Chinese herbs, yoga, and meditation. But there hasn’t been a lot of research into how well they work for psoriasis. Check with your doctor before you try one. Yoga and meditation are likely safe, but herbs and supplements could mess with your medications.
Oral medications include methotrexate (Trexall), acitretin (Soriatane), cyclosporine (Neoral), apremilast (Otezla), and others. Oral prednisone (corticosteroid) is generally not used in psoriasis and may cause a disease flare-up if administered.

Topical therapy, including corticosteroids, calcipotriene (Dovonex), coal tar products, tazarotene (Tazorac) and anthralin (Anthra-Derm), is the mainstay of treatment for localized disease (Tables 2 and 3). While the use of emollients should be encouraged, they should be used selectively because many (e.g., products containing lactic acid or alpha-hydroxy acids) can be irritating to inflamed or broken skin.

There are three nature-based topical remedies that have shown positive effects on psoriasis: Oregon grape (10 percent) cream, avocado and vitamin B12 cream, and aloe (0.5 percent) cream. Research has shown that using Reliéva, a homeopathic cream containing Oregon grape extract, is effective and well-tolerated in patients with mild to moderate psoriasis. (9) Early research suggests that a proprietary cream containing avocado oil and vitamin B12 may decrease psoriasis symptoms. Several studies have shown that a cream containing 0.5 percent aloe vera extract is superior to a placebo with no negative side effects. (10)
