histopathology Oral lichen planus is a inflammatory condition that affects the inside of the mouth.  Signs and symptoms include patches of fine white lines and dots most commonly in the inside of the cheeks, gums, and/or tongue. Most people with lichen planus experience no to few symptoms, others may have painful sores or ulcers in the mouth. Severe lichen planus slightly increases the risk for oral cancer. Cutaneous lichen planus may resolve spontaneously within one to two years, although lichen planus affecting mucous membranes may be more persistent and resistant to treatment. Recurrences are common, even with treatment. Table 4 summarizes the treatment of nongenital cutaneous lichen planus lesions. High-potency topical corticosteroids are first-line therapy for cutaneous lichen planus.14–16 Oral antihistamines (e.g., hydroxyzine [Vistaril]) may be used to control pruritus. Hypertrophic lesions are treated with intralesional triamcinolone acetonide (Kenalog), 5 to 10 mg per mL injection (0.5 to 1 mL per 2-cm lesion).14

    Oral lichenoid stomatitis can occur within a few months or many years after amalgam placement. The therapy for this form of contact dermatitis is to remove the amalgam and replace with a ceramic or other type of nonmetal dental restoration. This is best diagnosed by conventional patch testing. Histopathologic evaluation may show some subtle differences, although this is still debated by expert dermatopathologists.

    We zien meerdere platte paars-rode bultjes die vooral voorkomen op de binnenkant van de polsen en de enkels, maar ook op de onderrug, en de nek. Op de scheenbenen en enkels ziet men soms dikke verharde plekken. Lichen planus kan echter op alle plaatsen van het lichaam voorkomen, zelfs op het behaarde hoofd. In ongeveer 20% van de gevallen geven de afwijkingen vrijwel geen klachten en zijn ze bovendien gering van omvang. In de andere 80% is jeuk de meest voorkomende klacht. De jeuk kan zeer hevig zijn. Lichen planus toont de zogenaamde “Koebner fenomeen”. Op de plaatsen waar hevig gekrabd is of een wondje is geweest ontstaat een typische lichen planus afwijking. Een vervelende plaats om lichen planus te hebben vormt de slijmvliezen. Vooral in de mond, aan de binnenkant van de wangen, op het tandvlees of de tong kan lichen planus zitten, vaak zonder dat men zich dat bewust is. We zien dan fijne witte lijntjes en stipjes. Vaak worden deze plekjes voor het eerst bij toeval opgemerkt tijdens een tandartscontrole of door de dermatoloog. Veel ongemak geven ook lichen planus plekken op de penis (eikel) en de vagina. Niet zelden gaan deze plekjes over in pijnlijke wondjes en zweertjes. In ongeveer 10% van lichen planus zijn ook de nagels aangetast. De veranderingen bestaan uit in de lengterichting verlopende ribbeltjes, gleufjes en spleten. In ernstige gevallen kan de nagel er zelfs afvallen en niet meer terugkomen.

    It has been my good fortune to be able to report this extremely rare and interesting case. The rarity of the disease and the multiform character of the lesions are of sufficient interest in themselves. In addition, the case serves as a corroboration to one of the conclusions reached in a previous article,1 namely, that lichen planus and lichen acuminatus are closely related to one another. One of the arguments on which this conclusion is based is the comparative frequency with which the two diseases occur at the same time in the same person.

    Those with mild scattered asymptomatic disease may be best left untreated as many cases will spontaneously resolve. It is always a good idea to ask about new medications as many may induce LP.
